木瓜蛋白酶與石膏制動建立兔膝骨關節(jié)炎模型的比較

【摘要】:目的比較木瓜蛋白酶關節(jié)腔注射與關節(jié)石膏伸直位制動建立兔膝骨關節(jié)炎模型的效果。方法將21只實驗兔采用隨機區(qū)組分組法分為正常組、木瓜蛋白酶組及石膏組,每組7只。木瓜蛋白酶組予6%木瓜蛋白酶生理鹽水溶液于實驗開始第1、4、7天行右膝關節(jié)腔注射,注射結(jié)束4周后造模完成;石膏組予膝關節(jié)伸直位固定6周;記錄實驗期間動物不良情況發(fā)生率,造模完成后處死實驗兔,取軟骨標本進行大體觀察、Pelletier評分、Mankin's評分,免疫組化測定軟骨Ⅱ型膠原蛋白含量,ELISA法檢測血清和關節(jié)液中腫瘤壞死因子-α(TNF-α)、軟骨寡聚基質(zhì)蛋白(COMP)以及基質(zhì)金屬蛋白酶-3(MMP-3)的含量。結(jié)果大體觀察顯示木瓜蛋白酶組與石膏組均出現(xiàn)關節(jié)腫脹、關節(jié)間隙變窄、軟骨缺損、滑膜增生硬化、炎癥改變、骨贅形成;Pelletier評分、Mankin's評分、Ⅱ型膠原蛋白含量、血清和關節(jié)液中TNF-α、COMP以及MMP-3的含量,木瓜蛋白酶組、石膏組與正常組比較,差異均有統(tǒng)計學意義(P0.05);木瓜蛋白酶組與石膏組比較,上述指標差異均無統(tǒng)計學意義(P0.05)。木瓜蛋白酶組動物不良情況發(fā)生率明顯低于石膏組,差異有統(tǒng)計學意義(P0.01)。結(jié)論木瓜蛋白酶關節(jié)腔注射與石膏關節(jié)伸直位制動均能成功建立膝骨關節(jié)炎模型,但木瓜蛋白酶關節(jié)腔注射法更為快捷、簡便,并發(fā)情況少,可行性好。
