自體半腱肌腱重建距跟骨間韌帶動物模型

【摘要】:目的:利用自體半腱肌腱移植建立距跟骨間韌帶重建的動物模型。方法:將72只新西蘭白兔隨機分為重建組、切斷組和對照組。分別于4周、8周、16周、32周取材,對3個實驗組術前術后體重變化,重建韌帶的影像學、組織學和生物力學結果以及術后距下關節(jié)的活動范圍進行評估。結果:術后8周以后實驗動物體重變化出現差別,韌帶切斷組較重建組體重多增加0.32 kg(P=0.04);影像學、組織學和生物力學均支持移植韌帶成活,生物力學研究發(fā)現韌帶牽拉時出現2個張力屈服點;術后3組距下關節(jié)冠狀面(P0.05)和水平面(P0.001)活動度差異有統(tǒng)計學意義。結論:利用自體半腱肌腱移植重建距跟骨間韌帶可有效恢復距下關節(jié)的解剖和功能。移植韌帶經過壞死、塑形改建、成熟的過程與骨道形成末端結構愈合。對于研究距下關節(jié)穩(wěn)定性、韌帶斷裂后關節(jié)的繼發(fā)性改變、韌帶移植重建的轉歸提供了良好的動物模型材料。
[Abstract]:Objective: to establish an animal model of reconstruction of intercalcaneal ligament with semitendinosus tendon autograft. Methods: 72 New Zealand white rabbits were randomly divided into reconstruction group, transection group and control group. The changes of body weight before and after operation, the imaging, histological and biomechanical results of the reconstructed ligaments and the range of movement of the subtalar joint in the three experimental groups were evaluated. Results: after 8 weeks of operation, there were differences in the weight changes of the experimental animals. The weight of ligamentotomy group was 0.32 kg / kg, and the survival of graft ligament was supported by imaging, histology and biomechanics. The biomechanical study showed that there were two tension yield points in ligamentum ligamentum. There were statistically significant differences in the range of motion between the three groups (P 0.05) and P 0.001). Conclusion: reconstruction of the intercalcaneal ligament with autogenous semitendinosus tendon can effectively restore the anatomy and function of the subtalar joint. It provides a good animal model material for studying the stability of subtalar joint, the secondary change of joint after ligament rupture and the outcome of ligament graft reconstruction.
