炎癥性腸�。↖nflammatory bowel disease,IBD）是一組病因尚不明確的慢性復(fù)發(fā)性胃腸道疾病的統(tǒng)稱,主要分為潰瘍性結(jié)腸炎（ulcerative colitis,UC）和克羅恩�。–rohn’s disease,CD）兩大類。近年來(lái)的流行病學(xué)報(bào)道顯示,IBD的發(fā)病率呈逐年上升的趨勢(shì),已經(jīng)成為嚴(yán)重影響人類健康的常見(jiàn)腸道疾病之一。近期多項(xiàng)長(zhǎng)期隨訪研究發(fā)現(xiàn)抑郁、焦慮等心理應(yīng)激事件可增加非活動(dòng)期IBD復(fù)發(fā)的風(fēng)險(xiǎn),促進(jìn)腸道炎癥。從病理生理學(xué)角度考慮,機(jī)體對(duì)應(yīng)激的生理反應(yīng)是激活下丘腦-腺垂體-腎上腺皮質(zhì)軸,釋放促腎上腺皮質(zhì)激素釋放激素（corticotropin-releasing hormone,CRH）,其中CRH作為機(jī)體應(yīng)激反應(yīng)中的重要調(diào)節(jié)因子,通過(guò)中樞或外周受體調(diào)節(jié)內(nèi)分泌、神經(jīng)、免疫系統(tǒng)以及腸道等器官以應(yīng)答應(yīng)激所造成的刺激。既往研究早已證實(shí)除了中樞系統(tǒng)之外,CRH受體也廣泛存在于外周組織,包括十二指腸、小腸及結(jié)腸等消化器官。最近的動(dòng)物實(shí)驗(yàn)證實(shí)外周注射CRH可對(duì)十二指腸黏膜造成與母愛(ài)剝奪相似的應(yīng)激效果。因此,上述多項(xiàng)研究均提示CRH通過(guò)外周機(jī)制與腸道黏膜功能障礙的發(fā)生發(fā)展...

IBD 患病病程 6.5 ±5.4 年，相對(duì)于輕間進(jìn)行吸煙史、手術(shù)史以及 IBD 相關(guān)。SS 對(duì)患者的認(rèn)知應(yīng)激進(jìn)行評(píng)估，旨在衡控制和超負(fù)荷的應(yīng)激程度。對(duì)兩組患者 組患者的 CPSS 評(píng)分顯著高于輕度/中8.9 ±3.8，P=0.039），提示應(yīng)激可能影水平與 IBD 患者腸道的炎癥水D 患者腸道疾病活動(dòng)度的相關(guān)性患者的應(yīng)激水平與 IBD 患者腸道疾病關(guān)性分析。

Compared to mild and moderate IBD group, severe IBD patients group showed asignificant increase of CPSS (P=0.039) and there was moderate relationship between theCPSS and Mayo Score (R=0.625, P=0.0015)從圖 2 我們可以看出，在 Spearman 等級(jí)相關(guān)性分析時(shí)，P=0.0015，說(shuō)明患者的應(yīng)激水平與腸道疾病活動(dòng)度存在相關(guān)關(guān)系。此外，其統(tǒng)計(jì)學(xué)指標(biāo) R=0.625，提示 CPSS評(píng)分與改良的 Mayo 評(píng)分之間存在著正相關(guān)關(guān)系，且兩者之間相關(guān)的密切程度為中度。即 CPSS 評(píng)分越高，改良的 Mayo 評(píng)分水平越高。2、應(yīng)激加重 IBD 患者腸道炎癥的內(nèi)鏡評(píng)分及 H&E 病理評(píng)分既往多項(xiàng)研究發(fā)現(xiàn)相對(duì)于無(wú)癥狀的IBD患者，有癥狀的IBD患者心理應(yīng)激水平顯著提高。為了進(jìn)一步驗(yàn)證應(yīng)激可以加重IBD患者的腸道炎癥，我們選擇了CPSS評(píng)分最低的6位IBD患者和CPSS評(píng)分最高的6位IBD患者，收集其腸鏡的內(nèi)鏡圖片及活檢組織進(jìn)行分析。
