【摘要】：背景:隨著社會(huì)經(jīng)濟(jì)的發(fā)展,人民生活方式改變,老齡化進(jìn)程加快,慢性非傳染性疾病(以下簡(jiǎn)稱(chēng)“慢病”)以其高患病率、高致死率、高致殘率、漫長(zhǎng)病程等特點(diǎn),對(duì)人類(lèi)健康、經(jīng)濟(jì)、社會(huì)造成了嚴(yán)重的危害。醫(yī)療資源分配不均與人民日益增長(zhǎng)的健康需求之間的矛盾,導(dǎo)致慢病管理工作成效不足�，F(xiàn)今物聯(lián)網(wǎng)技術(shù)的快速發(fā)展,及其在醫(yī)療領(lǐng)域的迅速滲透,給慢病控制帶來(lái)了一條新型的管理道路。目的:在生物-心理-社會(huì)醫(yī)學(xué)模式的指導(dǎo)下,基于國(guó)家分級(jí)診療制度推行的重要任務(wù)要求,利用物聯(lián)網(wǎng)技術(shù),建立一條有效、科學(xué)、合理的慢病健康管理策略。促進(jìn)慢病管理變“被動(dòng)管理”為“自主管理”,為患者提供優(yōu)質(zhì)、連續(xù)、全程的慢病管理服務(wù),促進(jìn)慢病患者生活質(zhì)量的提高;促進(jìn)醫(yī)療資源合理分配,規(guī)范就醫(yī)秩序,形成合理就醫(yī)格局,緩解大醫(yī)院就醫(yī)壓力,提高基層醫(yī)療機(jī)構(gòu)和醫(yī)生的衛(wèi)生服務(wù)能力。方法:通過(guò)文獻(xiàn)分析法了解我國(guó)慢病管理領(lǐng)域面臨的問(wèn)題和挑戰(zhàn)、國(guó)內(nèi)的政策環(huán)境以及國(guó)內(nèi)外慢病管理的經(jīng)驗(yàn),構(gòu)想并設(shè)計(jì)一種以高血壓、糖尿病為切入點(diǎn)基于物聯(lián)網(wǎng)技術(shù)的社區(qū)慢病健康管理策略;通過(guò)半結(jié)構(gòu)式訪談法進(jìn)行基于物聯(lián)網(wǎng)的社區(qū)慢病健康管理的需求分析,并根據(jù)需求分析和國(guó)內(nèi)外經(jīng)驗(yàn)建立具體的管理方案和策略;通過(guò)項(xiàng)目試點(diǎn)進(jìn)行實(shí)踐驗(yàn)證,然后采用訪談法對(duì)管理策略進(jìn)行初步評(píng)價(jià);最后運(yùn)用魚(yú)骨圖分析法探索基于物聯(lián)網(wǎng)的社區(qū)慢病健康管理面對(duì)的問(wèn)題及原因,提出針對(duì)性的建議。結(jié)果:以醫(yī)院醫(yī)療技術(shù)為支撐,以社區(qū)衛(wèi)生服務(wù)中心為重點(diǎn),以患者和家庭為服務(wù)主體,運(yùn)用相應(yīng)的感知終端,建立管理信息系統(tǒng)為慢病患者提供綜合的醫(yī)療服務(wù)。將線下醫(yī)療服務(wù)與“健康管理系統(tǒng)平臺(tái)”線上健康管理結(jié)合,具體管理策略包括建立健康檔案管理、健康分析評(píng)估、健康干預(yù)計(jì)劃、動(dòng)態(tài)跟蹤隨訪、健康咨詢(xún)、健康教育、家庭-社區(qū)-醫(yī)院聯(lián)動(dòng)雙向轉(zhuǎn)診等。結(jié)論:基于物聯(lián)網(wǎng)的社區(qū)慢病健康管理策略通過(guò)社區(qū)項(xiàng)目試點(diǎn),初步評(píng)價(jià)效果良好。患者基本能按照要求上傳數(shù)據(jù),對(duì)感知設(shè)備使用情況反映良好,積極在醫(yī)生和健康管理師的指導(dǎo)干預(yù)下,進(jìn)行藥物治療和飲食、運(yùn)動(dòng)等生活方式改善以及心理調(diào)整、健康教育等。由于項(xiàng)目試點(diǎn)時(shí)間不長(zhǎng),沒(méi)有進(jìn)行定量的管理效果評(píng)價(jià),定性評(píng)價(jià)患者和醫(yī)務(wù)人員滿意度評(píng)價(jià)總體良好。通過(guò)對(duì)基于物聯(lián)網(wǎng)的慢病管理存在問(wèn)題的原因分析,從制度規(guī)范、經(jīng)濟(jì)、人員、管理策略、技術(shù)等方面提出針對(duì)性建議,以期促進(jìn)慢病管理的持續(xù)有效發(fā)展。
[Abstract]:Background: with the development of social economy, people's lifestyle changes, the aging process is accelerated, chronic non-communicable diseases (hereinafter referred to as "chronic diseases") for its high prevalence, high mortality, high disability rate. The long course of disease has caused serious harm to human health, economy and society, and the contradiction between the uneven distribution of medical resources and the growing health needs of the people. The current rapid development of Internet of things technology and its rapid penetration in the medical field. Under the guidance of biological psycho-social medical model and based on the important task requirements of the national hierarchical diagnosis and treatment system the use of Internet of things technology. To establish an effective, scientific and reasonable health management strategy for chronic illness, to promote the change of chronic disease management from "passive management" to "independent management", and to provide patients with high quality, continuous and complete chronic disease management services. To improve the quality of life of patients with chronic diseases; To promote the rational distribution of medical resources, standardize the order of medical treatment, form a reasonable pattern of medical treatment, ease the pressure of large hospitals to seek medical treatment. Methods: through the literature analysis to understand the problems and challenges in the field of chronic disease management, the domestic policy environment and the experience of chronic disease management at home and abroad. To conceive and design a community health management strategy based on Internet of things (IOT) based on hypertension and diabetes. The demand analysis of community chronic health management based on the Internet of things was carried out by semi-structured interview method, and the concrete management plan and strategy were established according to the needs analysis and experience at home and abroad. Through the project pilot to verify the practice, and then the use of interviews to carry out preliminary evaluation of the management strategy; Finally, fish bone map analysis was used to explore the problems and causes of community chronic disease health management based on the Internet of things, and put forward targeted suggestions. Results: hospital medical technology as the support. The community health service center as the focus, patients and families as the main service, the use of the corresponding perception terminal. To establish a management information system for chronic patients to provide comprehensive medical services. Offline medical services and "health management system platform" online health management, the specific management strategies include the establishment of health records management. Health analysis and evaluation, health intervention plan, dynamic follow-up, health counseling, health education. Conclusion: the community health management strategy based on the Internet of things through the community project pilot, preliminary evaluation results are good, patients can basically upload data according to requirements. Good response to the use of sensory equipment, actively under the guidance of doctors and health management intervention, drug treatment and diet, exercise and other lifestyle improvement and psychological adjustment. Health education and so on. Because the project pilot time is not long, did not carry on the quantitative management effect appraisal. Qualitative evaluation of patients and medical staff satisfaction evaluation is good. Through the analysis of the causes of the problems in the management of chronic diseases based on the Internet of things, from the system norms, economic, personnel, management strategies. In order to promote the sustainable and effective development of chronic disease management, technical suggestions are put forward.
