【摘要】：目的:通過觀察60例重癥肺炎患兒體內(nèi)Ig A、Ig M、Ig G及CD3~+、CD4~+、CD8~+、Th1、Th2、Th17、IFN-γ、IL-4、IL-17的變化,分析其病時(shí)的免疫功能狀態(tài),探尋其臨床意義,從而為小兒重癥肺炎的免疫學(xué)治療提供依據(jù)。方法:選取2016年1月-12月長(zhǎng)春市兒童醫(yī)院收治的確診為重癥肺炎的患兒60例作為觀察組,另選60例確診為輕癥肺炎且與觀察組同期收治的患兒為對(duì)照組。免疫散射比濁法和流式細(xì)胞術(shù)以及ELISA分析法觀察60例重癥肺炎患兒和對(duì)照組外周血中Ig A、Ig M、Ig G及CD3~+、CD4~+、CD8~+、Th1、Th2、Th17、IFN-γ、IL-4、IL-17的變化情況。結(jié)果:觀察組Ig A、Ig M、Ig G含量均出現(xiàn)不同程度的降低;CD3~+、CD4~+、CD8~+含量降低,CD4~+/CD8~+比值下降;Th1、Th17的水平均高于對(duì)照組,Th1/Th2比值高于對(duì)照組;觀察組IL-17含量明顯高于對(duì)照組。結(jié)論:患重癥肺炎小兒的Ig A、Ig G、Ig M含量更低;患重癥肺炎小兒的T淋巴細(xì)胞亞群水平降低;患重癥肺炎小兒的Th1/Th2失衡,存在更嚴(yán)重的免疫功能紊亂;Th17細(xì)胞參與了小兒重癥肺炎的病情進(jìn)展。
[Abstract]:Objective: to observe the changes of Ig mg G and CD3~, CD4~, CD8~, Th1,Th2,Th17,IFN- 緯, IL-4,IL-17 in 60 children with severe pneumonia, analyze their immune function and explore their clinical significance, so as to provide basis for immunological treatment of severe pneumonia in children. Methods: 60 children with severe pneumonia admitted in Changchun Children's Hospital from January to December 2016 were selected as observation group and 60 children with mild pneumonia as control group. The changes of Ig mg G and CD3~, CD4~, CD8~, Th1,Th2,Th17,IFN- 緯, IL-4,IL-17 in peripheral blood of 60 children with severe pneumonia and control group were observed by immunoturbidimetry, flow cytometry and ELISA assay. Results: in the observation group, the contents of Ig, CD4~ and CD8~ decreased, the ratio of CD4~ / CD8~ decreased, the Th1,Th17 level was higher than that of the control group, the Th1/Th2 ratio was higher than that of the control group, and the IL-17 content of the observation group was significantly higher than that of the control group. Conclusion: the content of Ig Ag G M in children with severe pneumonia is lower than that in children with severe pneumonia, the level of T lymphocyte subsets in children with severe pneumonia is lower, and the Th1/Th2 imbalance in children with severe pneumonia is lower than that in children with severe pneumonia. There are more severe immune disorders; Th17 cells are involved in the progression of severe pneumonia in children.
