【摘要】：目的通過觀察較大腎上腺腺瘤(4cm)患者行后腹腔鏡手術(shù)與開放手術(shù)的方法比較,探討適宜的手術(shù)方式。方法選取2012~2016年期間于青海大學(xué)附屬醫(yī)院及青海省人民醫(yī)院及就診的符合診斷較大腎上腺腺瘤(4cm),包括腎上腺皮質(zhì)醇腺瘤、嗜鉻細(xì)胞瘤、醛固酮腺瘤以及髓質(zhì)脂肪瘤患者,并行手術(shù)切除者57例,將入組的57例患者根據(jù)手術(shù)方案差異分為兩組,A組為行后腹腔鏡手術(shù)患者23例(其中2例中轉(zhuǎn)開放未算在統(tǒng)計(jì)資料內(nèi)),B組為行開放手術(shù)患者34例。對(duì)比分析兩組的手術(shù)時(shí)間、術(shù)中出血量、術(shù)后負(fù)壓管拔出時(shí)間以及術(shù)后住院時(shí)間。結(jié)果 A組在術(shù)后負(fù)壓管拔出時(shí)間、術(shù)中出血量、手術(shù)時(shí)間及術(shù)后住院時(shí)間方面均優(yōu)于B組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論應(yīng)用后腹腔鏡手術(shù)治療較大腎上腺腺瘤可良好改善患者圍術(shù)期狀況,能夠縮短手術(shù)時(shí)間,減少術(shù)中出血量,縮短術(shù)后負(fù)壓引流管拔出時(shí)間及術(shù)后住院時(shí)間,促進(jìn)預(yù)后恢復(fù)時(shí)間。后腹腔鏡手術(shù)治療腎上腺較大腺瘤(4cm)相對(duì)開放手術(shù)而言是一種更加安全、有效的微創(chuàng)手術(shù)方法。
[Abstract]:Objective to compare the methods of retroperitoneal laparoscopic surgery and open surgery in patients with large adrenal adenoma (4cm). Methods patients with large adrenal adenoma (4cm), including adrenocortisol adenoma, pheochromocytoma, aldosterone adenoma and medullary lipoma, were selected from Qinghai University affiliated Hospital and Qinghai Provincial people's Hospital from 2012 to 2016. According to the difference of operation plan, 57 patients were divided into two groups: group A (23 cases) undergoing retroperitoneal laparoscopic surgery (2 cases were not included in the statistical data) and group B (34 cases) underwent open surgery. The time of operation, the amount of blood lost during operation, the time of pulling out the negative pressure tube and the time of hospitalization after operation were compared between the two groups. Results the time of pulling out the negative pressure tube, the amount of blood lost during operation, the time of operation and the time of hospitalization after operation in group A were better than those in group B, and the difference was statistically significant (P0.05). Conclusion Laparoscopic surgery for large adrenal adenoma can improve the perioperative condition, shorten the operative time, reduce the amount of blood loss, shorten the time of pulling out the negative pressure drainage tube and the time of hospitalization after operation. Promote prognosis recovery time. Retroperitoneal laparoscopic surgery for adrenal adenoma (4cm) is a safer and more effective minimally invasive procedure than open surgery.
