泄瀉患者證候相關(guān)因素分析

【摘要】:目的:本研究采用流行病調(diào)查方法,對慢性泄瀉患者一般情況及飲食情況、情志狀態(tài)、工作情況等進行調(diào)查,分析探討泄瀉證型和影響因素之間的關(guān)系,通過分析泄瀉患者證型與不同影響因素之間相關(guān)性,旨在能給臨床醫(yī)生對泄瀉的辨證分型、診治和防治以及患者生活質(zhì)量的改善提供一定的理論依據(jù)和參考指導(dǎo)。方法:文獻(xiàn)研究整理了中醫(yī)和現(xiàn)代醫(yī)學(xué)對于泄瀉的認(rèn)識的相關(guān)文獻(xiàn),歸納、總結(jié)出影響泄瀉證型形成的影響因素,并構(gòu)建慢性泄瀉患者調(diào)查表,在廣州中醫(yī)藥大學(xué)脾胃病門診和普通門診開展問卷調(diào)研,運用SPSS22.0統(tǒng)計軟件進行數(shù)據(jù)統(tǒng)計分析:1.描述性分析,對調(diào)查對象性別、年齡、職業(yè)等的分布情況進行統(tǒng)計描述;2.Kruskal-Wallis H秩和檢驗和卡方檢驗:對肥甘厚味食物攝入情況、肉食攝入情況、工作時間的關(guān)系進行分析;3.多因素logistics回歸分析:用logistics回歸分析對泄瀉證型差異有統(tǒng)計學(xué)意義的影響因素進行分析。結(jié)果:通過對101例功慢性泄瀉患者86人,慢性泄瀉急性發(fā)作15人的一般情況、飲食情況、情志狀態(tài)、工作情況等調(diào)查顯示:經(jīng)常攝入肥甘厚味食物對于腸道濕熱型或食滯腸胃型泄瀉的形成有影響,χ~2=14.40,P0.001,各證型間的肉類食品攝入有統(tǒng)計學(xué)差異;經(jīng)常攝入肉類食物對于食滯腸胃型泄瀉的形成有影響,χ~2=14.40,P=0.006,各證型間的肉類食品攝入有統(tǒng)計學(xué)差異;經(jīng)常喝酒對于腸道濕熱型泄瀉的形成的有影響,χ~2=12.98,P=0.011,各證型間的飲酒情況有統(tǒng)計學(xué)差異;平時情志狀況對于肝氣郁滯型泄瀉的形成有影響,χ~2=11.27,P=0.024,各證型間平時的情志狀況有統(tǒng)計學(xué)差異(p0.05);工作時間對于肝氣郁滯型泄瀉的形成有影響,χ~2=10.62,P=0.031,各證型間的工作時間有統(tǒng)計學(xué)差異;失眠對于肝氣郁滯型泄瀉和脾氣虧虛型泄瀉的形成有影響,χ~2=35.01,P0.001,各證型間的失眠情況有統(tǒng)計學(xué)差異(p0.05)。結(jié)論:經(jīng)常攝入肥甘厚味食物對于形成腸道濕熱型或食滯腸胃型的泄瀉有影響;經(jīng)常攝入肉類食物對于食滯腸胃型的泄瀉有影響;經(jīng)常喝酒對于腸道濕熱型泄瀉的形成的有關(guān)系;平時情志狀況對于肝氣郁滯型泄瀉的形成有影響;工作時間對于肝氣郁滯型泄瀉的形成有影響;失眠對于肝氣郁滯型泄瀉和脾氣虧虛型泄瀉的形成有影響。綜上所得,居住在廣州地區(qū)的慢性泄瀉患者應(yīng)減少甚至避免肥甘厚味的攝入,平時控制好肉類食物的攝入,減少酒精類飲品的攝入,盡量保持心情舒暢,控制好工作時間,盡可能提高睡眠質(zhì)量,這些舉措都有可能有減少泄瀉患者的發(fā)病頻率和減輕發(fā)病時的癥狀。同時臨床醫(yī)生在對于泄瀉患者的辨證論治時也可以適當(dāng)參考
[Abstract]:Objective: to investigate the general and dietary status, emotional status, work status of patients with chronic diarrhea by epidemiological survey, and to analyze the relationship between the type of diarrhea and the influencing factors. By analyzing the correlation between the syndrome types of patients with diarrhea and different influencing factors, the purpose of this paper is to provide some theoretical basis and reference guidance for clinicians in the differentiation of symptoms and signs, diagnosis, treatment, prevention and treatment, and improvement of patients' quality of life. Methods: literature review was conducted on the knowledge of diarrhea in TCM and modern medicine. The factors influencing the formation of diarrhea syndrome were summarized, and the questionnaire for patients with chronic diarrhea was constructed. A questionnaire survey was conducted in the outpatient and general outpatient department of Guangzhou University of traditional Chinese Medicine, and the data were analyzed by SPSS22.0 software: 1. 1. Descriptive analysis. The distribution of sex, age and occupation were analyzed. 2. Kruskal-Wallis H rank sum test and chi-square test. The relationships among fat and sweet food intake, meat intake and working hours were analyzed. Multivariate logistics regression analysis: logistics regression analysis was used to analyze the influencing factors of the type of diarrhea. Results: in 101 cases of chronic diarrhea, 86 patients with chronic diarrhea and 15 patients with acute episode of chronic diarrhea were investigated. The investigation of work conditions showed that regular intake of fat and thick taste food had an effect on the formation of diarrhea of intestinal dampness and heat type or stomach stagnation type, 蠂 ~ (2 +) (蠂 ~ (2) ~ (14) 40 ~ (th) P _ (0.001), and there was significant difference in the intake of meat food among different syndrome types. Regular intake of meat food had an effect on the formation of gastroenteric diarrhea, 蠂 ~ 2 ~ 2 ~ (14. 40) P ~ (0.006), and there was a significant difference in the intake of meat food among different syndrome types. Frequent drinking had an effect on the formation of diarrhea of intestinal dampness and heat type, 蠂 ~ (2 +) 212.98% P0. 011, and there was statistical difference among different syndrome types. The normal emotional status had an effect on the formation of diarrhea with stagnation of liver-qi, 蠂 ~ 2 ~ 2 ~ (11.27) / P ~ (0.024). There was a significant difference in the normal state of emotion among the syndrome types (p0.05), and the working time had an effect on the formation of diarrhea with stagnation of liver-qi, 蠂 ~ (2 +) + 10.62% P0.031, and the working time of each syndrome type was statistically different. Insomnia had an effect on the formation of diarrhea with stagnation of liver-qi and diarrhea with deficiency of temper, 蠂 ~ (2 +) = 35.01 ~ (1) P 0.001, and there was significant difference in insomnia among different syndromes (p0.05). Conclusion: regular intake of fat and thick taste food has influence on diarrhea of intestinal damp-heat type or stomach stagnation type, and regular intake of meat food has influence on diarrhea of intestinal and stomach stagnation type. Regular drinking has a relationship with the formation of intestinal dampness and heat type diarrhea; usually the emotional state has an effect on the formation of the type of diarrhea with stagnation of liver-qi, and the working time has an effect on the formation of the type of diarrhea of the type of stagnation of liver-qi. Insomnia has an effect on the formation of diarrhea with stagnation of liver-qi and diarrhea with deficiency of temper. To sum up, patients with chronic diarrhea living in the Guangzhou area should reduce or even avoid the ingestion of fat, sweet and thick taste, control the intake of meat food, reduce the intake of alcoholic drinks, try to keep their mood at ease, and control their working hours as far as possible. To improve sleep quality as much as possible, these measures may reduce the frequency of diarrhea patients and reduce symptoms. At the same time, the clinicians can also refer to the syndrome differentiation of patients with diarrhea.
