【摘要】：目的:本課題采用神闕灸聯(lián)合針刺的療法對腦卒中后便秘患者進行臨床療效觀察,并觀察神闕灸聯(lián)合針刺療法對兩組患者餐前空腹及餐后胃動素水平的影響,探討其作用機理。方法:將60例符合納入標準的患者,隨機分為治療組、對照組。治療組在基礎(chǔ)治療上行神闕灸聯(lián)合針刺治療,每日1次,6次1個療程,共治療2個療程。對照組在基礎(chǔ)治療上行針刺治療,每日1次,療程和周期同治療組。通過記錄兩組治療前、治療后及治療后一個月患者餐前空腹、餐后2h血清胃動素濃度、便秘癥狀積分,運用SPSS20.0軟件進行統(tǒng)計分析,比較兩種療法的差異。結(jié)果:1.兩組治療前空腹及餐后胃動素水平、便秘癥狀積分,經(jīng)統(tǒng)計分析無顯著性差異(p0.05)。2.兩組治療后餐后胃動素水平、便秘癥狀積分,經(jīng)統(tǒng)計分析具有顯著性差異(p0.05)。3.兩組治療后1個月餐后胃動素水平、便秘癥狀積分,經(jīng)統(tǒng)計分析具有顯著性差異(p0.05)。4.兩組治療后臨床療效比較:治療后,治療組總有效86.7%,對照組總有效率80.0%,治療組總有效率明顯高于對照組,兩組相比有顯著性差異(p0.05)。結(jié)論:1.神闕灸聯(lián)合針刺、單純針刺治療對腦卒中后便秘患者的治療均有效,且神闕灸聯(lián)合針刺改善便秘癥狀療效優(yōu)于單純針刺。2.神闕灸聯(lián)合針刺及單純針刺治療均可升高患者餐后胃動素水平,而治療組優(yōu)于對照組。3.從隨訪期的數(shù)據(jù)分析可以得出,兩種治療方法均有遠期療效,而神闕灸聯(lián)合針刺組遠期療效優(yōu)于針刺組。4.神闕灸聯(lián)合針刺治療腦卒中后便秘是行之有效的方法,可調(diào)節(jié)患者餐后胃動素水平,顯著降低便秘癥狀積分,改善便秘癥狀,有利于卒中的康復(fù)。
[Abstract]:Objective: to observe the clinical curative effect of Shenque moxibustion combined with acupuncture on patients with constipation after stroke, and to observe the effect of Shenque moxibustion combined with acupuncture therapy on the fasting and postprandial motilin levels in the two groups, and to explore its mechanism. Methods: 60 patients who met the inclusion criteria were randomly divided into treatment group and control group. In the treatment group, there were two courses of treatment, 6 times a day and 6 times a course of treatment. The control group was treated with uplink acupuncture once a day, the treatment course and cycle were the same as the treatment group. The serum motilin concentration and constipation symptom score were recorded before treatment, one month after treatment and one month after treatment. The difference between the two treatments was compared by using SPSS20.0 software. The result is 1: 1. There was no significant difference in fasting and postprandial motilin level and constipation symptom score between the two groups by statistical analysis. The postprandial motilin level and constipation symptom score were significantly different between the two groups by statistical analysis. The level of motilin and the score of constipation symptom were significantly different between the two groups after 1 month of treatment (p 0.05. 4). Comparison of clinical efficacy between the two groups: after treatment, the total effective rate in the treatment group was 86.7, the total effective rate in the control group was 80.00.The total effective rate in the treatment group was significantly higher than that in the control group, and there was a significant difference between the two groups (P 0.05). Conclusion 1. Shenque moxibustion combined with acupuncture, simple acupuncture treatment for patients with constipation after stroke are effective, and Shenque moxibustion combined with acupuncture to improve the efficacy of constipation symptoms than simple acupuncture .2. Shenque moxibustion combined with acupuncture and simple acupuncture can increase the postprandial motilin level of patients, but the treatment group is better than the control group. 3. From the data analysis of the follow-up period, it can be concluded that the two treatment methods have long-term effect, while Shenque moxibustion combined with acupuncture group has better long-term effect than acupuncture group .4. Shenque moxibustion combined with acupuncture is an effective method for the treatment of post-stroke constipation, which can regulate the postprandial motilin level, significantly reduce the score of constipation symptoms, improve constipation symptoms, and is beneficial to the rehabilitation of stroke.
