頭穴留針配合干預(yù)訓(xùn)練對(duì)孤獨(dú)癥兒童CARS和ABC影響的臨床研究

【摘要】:目的:本研究通過(guò)結(jié)合針刺與干預(yù)訓(xùn)練對(duì)孤獨(dú)癥兒童進(jìn)行康復(fù),利用二者的優(yōu)勢(shì),以期在臨床中獲得較傳統(tǒng)康復(fù)療法更好的療效,并初步探索該療法對(duì)孤獨(dú)癥兒童不同臨床癥狀的影響,為孤獨(dú)癥的治療提供新思路。方法:將符合標(biāo)準(zhǔn)的58例孤獨(dú)癥患兒隨機(jī)分為治療組30和對(duì)照組28例。兩組用同樣方法進(jìn)行語(yǔ)言行為訓(xùn)練、感覺(jué)統(tǒng)合訓(xùn)練等干預(yù)訓(xùn)練,訓(xùn)練由蘇州市博愛(ài)兒童醫(yī)院的專業(yè)人員進(jìn)行。治療組另進(jìn)行針刺治療,體穴單刺不留針,頭穴留針并帶針進(jìn)行康復(fù)訓(xùn)練。治療每日1次,一周休息2天,1個(gè)療程為1個(gè)月,持續(xù)治療4個(gè)療程,并于療程結(jié)束后2個(gè)月進(jìn)行隨訪。治療前后由兒童醫(yī)院的專業(yè)人員運(yùn)用兒童孤獨(dú)癥評(píng)定量表(CARS)和孤獨(dú)癥兒童行為量表(ABC)進(jìn)行評(píng)估,對(duì)比兩組臨床療效,隨訪時(shí)評(píng)估CARS并統(tǒng)計(jì)有效率。結(jié)果:①總體療效:治療后,兩組總有效率及兩組療效等級(jí)比較,具有顯著性差異(P0.05),治療組療效優(yōu)于對(duì)照組;②CARS量表得分:兩組患者治療后評(píng)分與治療前相比均降低(P0.05),且治療組降低幅度明顯大于對(duì)照組(P0.05);③ABC量表得分:兩組治療后評(píng)分較治療前均降低(P0.05)。對(duì)ABC各個(gè)因子進(jìn)行組間治療后獨(dú)立比較發(fā)現(xiàn),感覺(jué)能力、運(yùn)動(dòng)能力和生活自理能力在兩組間治療后得分上并沒(méi)有顯著差異(P㧐0.05),而交往能力和語(yǔ)言能力比較則差異顯著(P0.05)。④隨訪CARS評(píng)分和有效率與治療后基本持平。結(jié)論:①頭穴留針配合干預(yù)訓(xùn)練治療兒童孤獨(dú)癥有效,且療效明顯優(yōu)于傳統(tǒng)單純康復(fù)訓(xùn)練;②頭穴留針配合干預(yù)訓(xùn)練在語(yǔ)言能力和交往能力改善上明顯優(yōu)于單純康復(fù)訓(xùn)練。
[Abstract]:Objective: through combining acupuncture and intervention training to rehabilitate autistic children, using the advantages of the two, we can obtain better curative effect than traditional rehabilitation therapy in clinical, and explore the effect of this therapy on the different clinical symptoms of autistic children and provide new ideas for the treatment of autism. Method: 58. The children with autism were randomly divided into the treatment group (30) and the control group (28 cases). The two groups were trained by the same method of language behavior training, sensory integration training and other intervention training. The training was carried out by the professionals of Suzhou Bo love children's hospital. The treatment group was treated with acupuncture, body point single needling, needle retention and needle rehabilitation training. 1 times, 2 days a week, 1 courses for 1 months, continuous treatment of 4 courses, and followed up for 2 months after the end of the course. Before and after the treatment, the professional staff of the children's hospital were evaluated by the children autism Assessment Scale (CARS) and the autistic child behavior scale (ABC), compared to the two groups of clinical effects, and the follow-up evaluation of CARS and the statistics were effective. Results: (1) total curative effect: after treatment, the total effective rate and two groups of curative effect grades were compared between the two groups (P0.05). The curative effect of the treatment group was better than the control group; (2) the score of the CARS scale: the scores of the two groups were lower than those before the treatment (P0.05), and the reduction of the treatment group was significantly greater than that of the control group (P0.05); (3) the ABC scale score: After treatment, the scores of the two groups were lower than those before the treatment (P0.05). There was no significant difference (P? 0.05) between the two groups after the treatment of each factor between the two groups (P? 0.05), while the communication ability and the language ability were significantly different (P0.05). (4) follow up CARS score and there was a follow-up The efficiency is basically the same as that after treatment. Conclusion: first, the head hole needle retention combined with intervention training is effective in the treatment of autism in children, and the curative effect is obviously better than the traditional simple rehabilitation training; 2. The acupuncture at the head point combined with intervention training is obviously better than the simple rehabilitation training in the improvement of language ability and communication ability.
