【摘要】：目的探討乳腺癌患者心理韌性保護(hù)性因素的相互作用關(guān)系,構(gòu)建心理韌性保護(hù)性因素的結(jié)構(gòu)模型,明確心理韌性保護(hù)性因素的作用路徑,為臨床醫(yī)護(hù)人員采取干預(yù)措施,提高乳腺癌患者的心理適應(yīng)能力,改善其生活質(zhì)量提供理論依據(jù);方法采用橫斷面研究,調(diào)查了2015年10月至2016年8月于安徽省某三甲醫(yī)院乳腺外科400例乳腺癌患者,采用一般資料問(wèn)卷、心理韌性量表、一般自我效能量表、生活定向量表修訂版、自我掌控感量表、多元領(lǐng)悟社會(huì)支持量表進(jìn)行調(diào)查。數(shù)據(jù)使用Epi Data3.1軟件進(jìn)行錄入,SPSS13.0統(tǒng)計(jì)軟件進(jìn)行人口學(xué)資料、各量表得分和相關(guān)系分析,使用Amos17.0軟件進(jìn)行驗(yàn)證性因子分析、結(jié)構(gòu)模型的構(gòu)建以及中介效應(yīng)分析。結(jié)果(1)乳腺癌患者心理韌性、一般自我效能感、自我掌控感、樂(lè)觀傾向、領(lǐng)悟社會(huì)支持量表總得分分別為78.81±11.53、26.65±5.08、27.91±3.96、18.01±3.43,72.51±8.12,項(xiàng)目均分分別為5.63±0.83、2.66±0.51、3.99±0.57、3.00±0.57、6.04±0.68;(2)相關(guān)性分析:心理韌性總分與一般自我效能感、樂(lè)觀傾向、自我掌控感、社會(huì)支持及三個(gè)維度的相關(guān)性系數(shù)在0.198~0.893(p0.05)之間;(3)驗(yàn)證性因子分析發(fā)現(xiàn)本研究中使用的量表各項(xiàng)指標(biāo)適配指標(biāo)均較好(GFI,AGFI,IFI值多數(shù)大于0.90,RMSEA值均小于0.08);(4)分別以領(lǐng)悟社會(huì)支持、一般自我效能感、自我掌控感、樂(lè)觀傾向?yàn)橹薪樽兞繕?gòu)建結(jié)構(gòu)模型,發(fā)現(xiàn)四個(gè)結(jié)構(gòu)模型的適配度均達(dá)到標(biāo)準(zhǔn)。結(jié)論本研究構(gòu)建的乳腺癌患者心理韌性保護(hù)性因素相互作用模型,均具有較好的適配指標(biāo)。一般自我效能感、樂(lè)觀傾向、自我掌控感、領(lǐng)悟社會(huì)支持對(duì)心理韌性產(chǎn)生直接的保護(hù)性作用,同時(shí)可以通過(guò)其中任一個(gè)心理韌性保護(hù)性因素作為中介變量對(duì)心理韌性產(chǎn)生間接的預(yù)測(cè)效應(yīng)。
[Abstract]:Objective to explore the interaction of protective factors of psychological toughness in patients with breast cancer, to construct the structural model of protective factors of psychological toughness, to clarify the action path of protective factors of psychological toughness, and to take intervention measures for clinical medical staff. Methods A cross-sectional study was conducted to investigate 400 breast cancer patients from October 2015 to August 2016 in a breast surgery department in Anhui Province. General information questionnaire, mental resilience scale, general self-efficacy scale, life orientation scale, self-control scale, The data were recorded by Epi Data3.1 software and SPSS 13.0 statistical software for demographic data. The scores of each scale and their relationships were analyzed, and the confirmatory factor analysis was performed by Amos17.0 software. Results 1) Psychological resilience, general self-efficacy, self-control, optimistic tendency of breast cancer patients. The total scores of perceived social support scale were 78.81 鹵11.53 鹵26.65 鹵5.0827.91 鹵3.960.91 鹵3.43n 18.01 鹵3.43 鹵72.51 鹵8.12, respectively, and the average scores of items were 5.63 鹵0.83n 2.66 鹵0.51n 3.99 鹵0.576.00 鹵0.576.04 鹵0.682.00) correlation analysis: the total score of psychological resilience and general self-efficacy, optimistic tendency, self-control, The correlation coefficient of social support and the three dimensions was between 0.198 and 0.893p0.05) confirmatory factor analysis showed that all the indicators used in this study were better than 0.90 RMSEA, and the RMSEA were less than 0.08) in order to understand social support. General self-efficacy, self-control and optimism tend to be mediating variables to construct structural models, and it is found that the adaptation of the four structural models is up to the standard. Conclusion the protective factor interaction model of psychological toughness of breast cancer patients was constructed in this study. General self-efficacy, optimism, self-control, social support have direct protective effect on psychological resilience. At the same time, one of the protective factors of psychological toughness can be used as an intermediate variable to indirectly predict the psychological toughness.
