【摘要】：目的:觀察漢族與維吾爾族患者慢性根尖周炎行一次法根管治療術后疼痛發(fā)生率及劇烈程度的差異,同時比較不同民族間布洛芬對于根管治療術后鎮(zhèn)痛效果的差異,為臨床治療提供參考。方法:本次臨床實驗分兩個階段,第一階段(預實驗)臨床實驗隨機選取漢族、維吾爾族慢性根尖周炎患者各30名。漢族患者年齡在16至39歲之間有28人,40至59歲之間有2人,其中男性11人,女性19人;維吾爾族患者年齡在16至39歲之間有27人,40至59歲之間有3人,其中男性15人,女性15人。采用冠向下法行根管治療,治療結束后記錄相關數(shù)據(jù),統(tǒng)計各組根管治療后疼痛(endodontic flare-up,E flare-up)發(fā)生率和急性發(fā)作指數(shù)(Flare-up index,FUI)。第二階段實驗隨機選取漢族、維吾爾族單根管慢性根尖周炎患者各250人,行一次法根管治療后,按照隨機雙盲的原則,將漢族患者術后分為實驗組(漢)和對照組(漢);將維吾爾族患者術后分為實驗組(維)、對照組(維)。實驗組和對照組出現(xiàn)疼痛的患者均按照每次一粒,每日兩次的服藥劑量,分別服用布洛芬緩釋膠囊和安慰劑(淀粉膠囊)三天。醫(yī)師與患者在整個實驗過程中,對于分組和服藥情況均不知情。利用可視化痛覺量表(visual analogue scale,VAS)記錄患者術后6小時、12小時、24小時、48小時、72小時和1周的疼痛程度。結果:第一階段(預實驗),漢族患者共有6人發(fā)生E flare-up,發(fā)生率為20.0%,FUI均值為0.75;維吾爾族患者共有10人發(fā)生E flare-up,發(fā)生率為33.3%,FUI均值為2.00。維吾爾族患者E flare-up的發(fā)生率和FUI均顯著高于漢族患者。第二階段實驗在時間分布上,根管治療后疼痛多發(fā)生于24至48小時之間,維吾爾族患者根管治療后疼痛的發(fā)生率和劇烈程度均更顯著。結論:慢性根尖周炎一次法根管治療后,維吾爾族患者術后疼痛發(fā)生率高于漢族患者;服用布洛芬后漢族患者的鎮(zhèn)痛效果更明顯。
[Abstract]:Objective: to observe the pain incidence and severity of chronic periapical periodontitis in Han and Uygur patients, and to compare the analgesic effect of ibuprofen after root canal therapy among different nationalities. Methods: the clinical trial was divided into two stages. There were 28 patients with chronic periapical periodontitis in Uygur nationality aged from 16 to 39 years old and 2 patients aged between 40 and 59 years, including 11 males and 19 females. There were 27 Uygur patients between 16 and 39 years of age and 3 between 40 and 59 years of age, including 15 men and 15 women. Root canal therapy was performed with the method of crown down, and relevant data were recorded after treatment. The incidence of pain and acute attack index Flareindex-up (Flareindex-up) after root canal therapy in each group were analyzed. In the second stage, 250 patients with chronic periapical periodontitis of single root canal of Uygur nationality were randomly selected. According to the principle of random double blindness, the patients of Han nationality were divided into experimental group (Han) and control group (Han) after operation. Take ibuprofen sustained-release capsule and placebo (starch capsule) twice a day for three days. Visual analogue scale was used to record the pain degree of patients 6 hours, 12 hours, 48 hours, 48 hours, 72 hours and 1 week after operation. Results: the first stage (pre-experiment, Han nationality). There were 6 cases of E flare-up. the incidence rate of E flare-upwas 20.0% and the mean value of E flare-upis was 0.75 in 10 Uygur patients, and the incidence rate of E flare-upwas 3.33%. The incidence of E flare-up and FUI in Uygur patients were significantly higher than those in the second stage of Uygur patients. In the time distribution of the experiment, The incidence and severity of pain after root canal therapy in Uygur patients were more significant. Conclusion: after the treatment of chronic periapical periodontitis, the incidence and severity of pain in Uygur patients were higher than those in Uygur patients. Conclusion: the incidence and severity of pain after root canal therapy are more significant in Uygur patients. The incidence of postoperative pain in Uygur patients was higher than that in Han patients, and the analgesic effect of ibuprofen was more obvious.
