【摘要】：目的:本研究以中醫(yī)基礎理論為指導原則,對于寒凝血瘀引起的原發(fā)性痛經,以暖宮寧心湯為基礎方,觀察治療前后經前或經期小腹冷痛拒按、面色蒼白、惡心嘔吐、頭暈乏力、冷汗淋漓、四肢厥冷、腰腿酸痛等癥狀的變化情況,從而客觀地評估暖宮寧心湯治療原發(fā)性痛經寒凝血瘀證的臨床療效,進一步探討該方法治療原發(fā)性痛經寒凝血瘀證的作用機制。方法:將原發(fā)性痛經寒凝血瘀證40例患者,予暖宮寧心湯治療,治療3個月經周期后,進行治療前后自身的對照,觀察治療前后痛經評分、寒凝血瘀癥候評分變化情況及討論暖宮寧心湯溫經散寒,祛瘀止痛的作用機制。結果:暖宮寧心湯治療原發(fā)性痛經寒凝血瘀證患者40例,痊愈10例(25.00%);顯效18例(45.00%);有效8例(20.00%);無效4例(10.00%),總有效率為90.00%。治療后與治療前相比較,痛經評分、寒凝血瘀癥候評分均明顯下降,差異有統(tǒng)計學意義(P0.05)。在整個治療過程中及治療停止后均沒有發(fā)現(xiàn)明顯過敏和不良反應。結論:內服暖宮寧心湯對原發(fā)性痛經寒凝血瘀證患者的痛經及中醫(yī)癥狀能夠有效地消除或緩解,而且毒副作用反應的發(fā)生率比較低,能很好地提高患者的生活質量,是一種即安全又有效的治療方法,在臨床上的使用范圍較為廣泛,值得臨床作進一步的推廣擴大應用,以及做更深入的研究。
[Abstract]:Objective: To study the TCM basic theory as the guiding principle, due to blood stasis primary dysmenorrhea, to warm house Ningxin Tang, observed before and after treatment in the period before or microgastropods Lengtong refused to press, pale, nausea and vomiting, dizziness, cold sweat, cold limbs, changes low back pain and other symptoms, so as to objectively evaluate the clinical efficacy of Nuangong Ningxin Decoction in the treatment of primary dysmenorrhea of congealing cold and blood stasis syndrome, the mechanism to further explore the methods for the treatment of primary dysmenorrhea of congealing cold and blood stasis syndrome. Methods: primary dysmenorrhea Hanning blood stasis syndrome in 40 patients, to warm Gong Ning the heart decoction treatment, treatment after 3 menstrual cycles, compared the before and after treatment, the dysmenorrhea score were observed before and after treatment, blood stasis symptom score changes and discuss Nuangong Ningxin Tang Wen and cold dispelling, mechanism of removing blood stasis and relieving pain. Results: Nuangong Ningxin Decoction on the treatment of primary Dysmenorrhea in patients with blood stasis in 40 cases, 10 cases were cured (25%); 18 cases (45%); 8 cases were effective (20%); 4 cases (10%), the total efficiency was 90.00%. after treatment compared with before treatment, dysmenorrhea score, blood stasis syndrome score decreased significantly, the difference there was a significant correlation (P0.05). Significant allergies and adverse reactions were not found in stopping the whole course of treatment and after treatment. Conclusion: oral Nuangong Ningxin Decoction on symptoms of primary dysmenorrhea dysmenorrhea and TCM syndrome of cold coagulation and blood stasis patients can effectively eliminate or alleviate the side effects, and reaction rate relatively low, can effectively improve the quality of life of patients, is a safe and effective treatment method used in clinical widely application, worthy of further promotion and expansion of clinical work, to do more in-depth study.
