【摘要】：目的:本研究運用補腎調(diào)周法配合耳壓治療多囊卵巢綜合征(PCOS)患者的臨床研究,通過比較治療組及對照組治療前后患者的月經(jīng)周期、多毛、痤瘡等臨床癥狀的改善情況,肥胖指數(shù)(BMI)、基礎(chǔ)體溫(BBT)、基礎(chǔ)血性激素及婦科B超的變化情況。評價補腎調(diào)周法配合耳壓的療效,探討耳壓對補腎調(diào)周法治療痰濕瘀阻型PC0S的潛在協(xié)同效用。方法:本研究選擇2015年02月至2017年02月就診于江蘇省中醫(yī)院婦科門診的多囊卵巢綜合征患者共60例,按照隨機原則分為兩組:(1)治療組:30例,予補腎調(diào)周法及耳壓配合口服達英-35治療;(2)對照組:30例,予耳壓配合口服達英-35治療。三個月經(jīng)周期為一個療程,觀察兩個療程治療前后患者的月經(jīng)周期、多毛、痤瘡等臨床癥狀的改善情況,肥胖指數(shù)(BMI)、基礎(chǔ)體溫(BBT)、基礎(chǔ)血性激素及婦科B超的變化情況。結(jié)果:綜合療效方面,治療組總有效率為83.33%,對照組總有效率為73.33%。兩組在臨床表現(xiàn)及中醫(yī)征候、BBT、BMI、基礎(chǔ)血性激素水平及B超方面治療前后均有改善(P0.05)。組間比較:臨床癥狀改善、BMI、T、DHEAS、SHBG下降率及B超情況改善方面治療組優(yōu)于對照組(P0.05),BBT改善情況及LH與LH/FSH下降率方面治療組與對照組組間差異無統(tǒng)計學意義(P0.05)。結(jié)論:本研究結(jié)合辨證論治的原則與月經(jīng)周期的規(guī)律,以針藥結(jié)合的方法,證實了調(diào)周法配合耳壓可明顯改善痰瘀互結(jié)型PC0S患者的臨床癥狀,療效確切,且臨床無明顯副作用。其作用機制可能是通過降低血清中睪酮和游離睪酮的濃度,降低血清促黃體生成素水平,從而調(diào)節(jié)內(nèi)分泌水平來實現(xiàn),可推廣運用于臨床,且可以為臨床提供有效的循證醫(yī)學證據(jù)。
[Abstract]:Objective: To study the application of tkrpm combined with ear pressure treatment of polycystic ovary syndrome (PCOS) patients with clinical research, through the comparison of treatment group and control to improve the patient's menstrual cycle, hirsutism and acne and other clinical symptoms before and after treatment, obesity index (BMI), basal body temperature (BBT), the change of foundation sex hormone and gynecological ultrasound. To evaluate the effect of Tonifying the kidney and regulating the week with the ear pressure, and to explore the potential synergistic effect of the ear pressure on the treatment of phlegm dampness and stasis type PC0S by the method of Tonifying the kidney and regulating the perimeter. Methods: a total of 60 cases of polycystic ovarian syndrome were selected in this study from 2015 02 to 2017 02 months in Jiangsu Province Traditional Chinese Medicine Hospital from gynecological outpatient clinic, were randomly divided into two groups: (1) the treatment group: 30 cases, treated with the method of invigorating kidney and ear pressure combined with oral treatment with -35; (2) control group: in 30 cases, treated with oral -35 treatment with ear pressure. The three menstrual cycle is a course of treatment. We observed the improvement of clinical symptoms, such as menstrual cycle, hairy and acne, and the changes of obesity index (BMI), basal body temperature (BBT), basal blood hormone and gynecological ultrasound before and after two courses of treatment. Results: the total effective rate of the treatment group was 83.33%, and the total effective rate of the control group was 73.33%. The two groups before and after treatment in the clinical manifestations and syndrome, BBT, BMI, sex hormone levels and ultrasound were improved (P0.05). Comparison between groups: the improvement of clinical symptoms, BMI, T, DHEAS, SHBG and the improvement of B-mode ultrasound were better in the treatment group than in the control group (P0.05). There was no significant difference in the improvement of BBT and the decrease rate of LH and LH/FSH between the treatment group and the control group (P0.05). Conclusion: combined with the principle of syndrome differentiation and menstrual cycle, this study confirms that the combination of acupuncture and medicine can significantly improve the clinical symptoms of patients with phlegm and blood stasis type PC0S, and has no obvious side effects. The mechanism may be achieved by decreasing the concentration of testosterone and free testosterone in serum, decreasing serum luteinizing hormone level and regulating endocrine level, which can be widely applied in clinical practice, and can provide effective evidence-based medical evidence for clinical practice.
