【摘要】：統(tǒng)籌城鄉(xiāng)發(fā)展是新時期黨和國家提出的一個新的發(fā)展戰(zhàn)略，這一發(fā)展戰(zhàn)略對當(dāng)前社會的醫(yī)療保障體制提出了嚴(yán)峻的挑戰(zhàn)。進(jìn)城農(nóng)民工作為我國經(jīng)濟(jì)社會發(fā)展的中堅(jiān)力量，已經(jīng)成為不可忽視的就業(yè)群體，需要社會化的醫(yī)療保障。在我國未富先老的情況下，老齡化趨勢的不斷加強(qiáng)給社會保障支出增加了巨大的壓力。同時，統(tǒng)籌城鄉(xiāng)發(fā)展要求社會保障擴(kuò)大覆蓋面，建立城鄉(xiāng)統(tǒng)一的社會保障體系。本文在城鄉(xiāng)統(tǒng)籌背景下，提出并設(shè)計了進(jìn)城農(nóng)民工醫(yī)療保險制度，并進(jìn)行了定量分析。 首先研究了城鄉(xiāng)統(tǒng)籌、進(jìn)城農(nóng)民工以及農(nóng)民工醫(yī)療保險的相關(guān)理論，分析了進(jìn)城農(nóng)民工醫(yī)療保險制度的模式，得出現(xiàn)存的進(jìn)城農(nóng)民工醫(yī)療保險模式取向不明確的結(jié)論。對我國進(jìn)城農(nóng)民工醫(yī)療保險制度以及城鄉(xiāng)統(tǒng)籌進(jìn)程中進(jìn)城農(nóng)民工醫(yī)療保險模式的國際經(jīng)驗(yàn)專門進(jìn)行了分析，指出了各自的優(yōu)缺點(diǎn)，以及對我國的借鑒作用。 然后，通過對上海市的實(shí)證調(diào)研，運(yùn)用統(tǒng)計軟件對調(diào)研數(shù)據(jù)進(jìn)行分析，得到進(jìn)城農(nóng)民工的主要特征。在此基礎(chǔ)上，提出并設(shè)計了我國當(dāng)前城鄉(xiāng)統(tǒng)籌階段進(jìn)城農(nóng)民工的醫(yī)療保險制度。在本文的制度設(shè)計中，根據(jù)進(jìn)城農(nóng)民工的市民化程度將進(jìn)城農(nóng)民工分為三大類，然后將這三類人群分別納入三類保險中，最終實(shí)現(xiàn)進(jìn)城農(nóng)民工醫(yī)療保障的城鄉(xiāng)統(tǒng)籌發(fā)展。 本文重視對于數(shù)據(jù)的定量分析。在分層次、分類表、有針對性構(gòu)建模型的基礎(chǔ)上，對模型進(jìn)行了模擬仿真運(yùn)算，運(yùn)用精算模型、系統(tǒng)動力學(xué)模型以及EXCEL表遞推法定量分析了城鄉(xiāng)居民基本醫(yī)療保險基金的收支問題。 本文的創(chuàng)新之處在于：第一，創(chuàng)新性地基于城鄉(xiāng)統(tǒng)籌發(fā)展的大趨勢下解決進(jìn)城農(nóng)民工醫(yī)療保險問題；第二，順應(yīng)社會發(fā)展的要求，，最大限度地做到了進(jìn)城農(nóng)民工的全覆蓋；第三，解決了上海市現(xiàn)存的社會保險制度的碎片化問題；最后，模式的設(shè)計和對模式的檢驗(yàn)具有很強(qiáng)的針對性，即不同的人群設(shè)計不同的模式，專門針對基金的收支采用了定量檢驗(yàn)的方法，體現(xiàn)了模式設(shè)計和方法檢驗(yàn)的科學(xué)性。
[Abstract]:Urban and rural development as a whole is a new development strategy put forward by the Party and the country in the new period. This development strategy poses a severe challenge to the current social medical security system. As the backbone of our country's economic and social development, migrant workers in cities have become an employment group that can not be ignored and need socialized medical security. In China, the trend of aging has increased the pressure on social security expenditure. At the same time, the overall development of urban and rural areas requires the expansion of social security coverage, the establishment of a unified urban and rural social security system. Under the background of urban and rural planning, this paper puts forward and designs the medical insurance system for migrant workers in cities, and carries out quantitative analysis. First of all, it studies the theories of urban and rural planning, migrant workers and migrant workers medical insurance, analyzes the mode of medical insurance system for migrant workers in cities, and draws the conclusion that the existing medical insurance mode of migrant workers in cities is not clear. This paper analyzes the medical insurance system of migrant workers in cities and the international experience of the medical insurance model of migrant workers in the process of urban and rural planning, and points out their advantages and disadvantages, as well as the reference to our country. Then, through the empirical investigation in Shanghai, the main characteristics of migrant workers in cities are obtained by using statistical software to analyze the survey data. On this basis, the medical insurance system for migrant workers in urban and rural areas is put forward and designed. In the system design of this paper, according to the degree of urbanization of migrant workers in cities, migrant workers are divided into three categories, and then these three groups of people are respectively included in the three types of insurance, and finally realize the urban and rural development of medical security for migrant workers in cities as a whole. This paper focuses on quantitative analysis of data. On the basis of hierarchical, classified table and targeted model, the model was simulated and simulated, and the actuarial model was used. The system dynamics model and EXCEL table recursive method are used to quantitatively analyze the income and expenditure of the basic medical insurance fund of urban and rural residents. The innovation of this paper lies in: first, based on the overall development of urban and rural areas to solve the problem of medical insurance for migrant workers; second, to comply with the requirements of social development, to maximize the full coverage of migrant workers in cities; Thirdly, the fragmentation of the existing social insurance system in Shanghai has been solved. Finally, the design of the model and the test of the model have very strong pertinence, that is, different people design different models, especially for the fund income and expenditure using the quantitative test method, which reflects the scientific nature of the mode design and method test.
