【摘要】：捐獻(xiàn)協(xié)調(diào)員在器官捐獻(xiàn)過(guò)程中發(fā)揮著舉足輕重的作用,隨著醫(yī)療制度的改革,越來(lái)越多的醫(yī)療機(jī)構(gòu)開(kāi)始設(shè)立醫(yī)務(wù)社會(huì)工作崗位,受過(guò)專(zhuān)業(yè)訓(xùn)練的醫(yī)務(wù)社會(huì)工作者(以下簡(jiǎn)稱(chēng)“醫(yī)務(wù)社工”),其知識(shí)結(jié)構(gòu)和能力更加匹配器官協(xié)調(diào)員的要求,因此越來(lái)越多的醫(yī)務(wù)社工通過(guò)培訓(xùn)、考核,進(jìn)入器官捐獻(xiàn)協(xié)調(diào)員隊(duì)伍。但由于器官捐獻(xiàn)涉及許多倫理問(wèn)題,而醫(yī)務(wù)社工有其專(zhuān)業(yè)的價(jià)值觀(guān)和倫理守則,使得醫(yī)務(wù)社工在開(kāi)展器官捐獻(xiàn)協(xié)調(diào)過(guò)程中,常會(huì)陷入各種倫理困境。本研究希望通過(guò)醫(yī)務(wù)社工在器官捐獻(xiàn)過(guò)程中的倫理困境研究,找出其倫理決策的相關(guān)因素及觀(guān)點(diǎn)。本研究目的如下:一、探討醫(yī)務(wù)社工在器官捐獻(xiàn)過(guò)程中,對(duì)于倫理困境的認(rèn)知、進(jìn)行倫理決策的因素。二、幫助醫(yī)務(wù)社工對(duì)器官捐獻(xiàn)過(guò)程中的倫理困境及決策有更清楚的自我認(rèn)知及專(zhuān)業(yè)掌握。三、通過(guò)研究,讓醫(yī)務(wù)社工了解自省個(gè)人生命經(jīng)驗(yàn)與價(jià)值觀(guān)對(duì)器官捐獻(xiàn)工作的影響。本研究旨在探討器官捐獻(xiàn)社會(huì)工作實(shí)務(wù)過(guò)程中醫(yī)務(wù)社工對(duì)相關(guān)倫理議題的察覺(jué),以及其決策的過(guò)程與因素。醫(yī)務(wù)社工在面對(duì)捐獻(xiàn)者及家屬、醫(yī)療團(tuán)隊(duì)、法律法規(guī)及社會(huì)文化等等的交互影響之下,到底哪些倫理議題容易使醫(yī)務(wù)社工產(chǎn)生困境?醫(yī)務(wù)社工又如何在有限的時(shí)間內(nèi),進(jìn)行相關(guān)決策?基于這個(gè)動(dòng)機(jī),本研究通過(guò)訪(fǎng)談資料收集的方法,訪(fǎng)談了在醫(yī)療機(jī)構(gòu)中從事器官捐獻(xiàn)的醫(yī)務(wù)社會(huì)工作者,從而達(dá)到對(duì)本研究的了解,讓研究者對(duì)器官捐獻(xiàn)社會(huì)工作實(shí)務(wù)工作與本研究議題有了更深的認(rèn)識(shí)和理解。研究發(fā)現(xiàn):(一)在維護(hù)捐贈(zèng)者知情同意權(quán)利的情境、捐獻(xiàn)者與受贈(zèng)者隱私的保護(hù)、專(zhuān)業(yè)角色和團(tuán)隊(duì)期待之間的沖突、醫(yī)務(wù)社工個(gè)人價(jià)值觀(guān)的沖突等情境中易使醫(yī)務(wù)社工產(chǎn)生倫理困境。(二)醫(yī)務(wù)社工進(jìn)行倫理決策的主要原則為堅(jiān)持保密原則、法律規(guī)定為根本、以案主及家屬利益為原則等。針對(duì)上述研究發(fā)現(xiàn),在最后的研究建議部分筆者提出,在個(gè)人方面,將倫理決策行動(dòng)過(guò)程列入個(gè)案記錄的一部分,同時(shí)通過(guò)不斷反思自身對(duì)生命的態(tài)度,審視個(gè)人價(jià)值觀(guān)的影響;在部門(mén)層面,通過(guò)督導(dǎo)方式提升器官捐獻(xiàn)新進(jìn)醫(yī)務(wù)社工的專(zhuān)業(yè)能力;在醫(yī)院層面,通過(guò)團(tuán)隊(duì)會(huì)議建立共識(shí)并信任專(zhuān)業(yè)間評(píng)估,同時(shí)建構(gòu)良好的機(jī)構(gòu)倫理氛圍環(huán)境;對(duì)社會(huì)大眾層面,在宣傳倡導(dǎo)器官捐獻(xiàn)的同時(shí)確保對(duì)捐獻(xiàn)者及家屬隱私的尊重。
[Abstract]:The coordinator of donation plays an important role in the process of organ donation. With the reform of medical system, more and more medical institutions begin to set up medical social work posts. The knowledge structure and ability of professionally trained medical social workers (hereinafter referred to as "medical social workers") are more consistent with the requirements of organ coordinators. Therefore, more and more medical social workers are trained and assessed. Enter the organ donation coordinator team. However, organ donation involves many ethical issues, and medical social workers have their professional values and codes of ethics, which makes medical social workers often fall into various ethical difficulties in the process of organ donation coordination. The purpose of this study is to find out the relevant factors and viewpoints of the ethical decision by studying the ethical dilemma of medical social workers in the process of organ donation. The purpose of this study is as follows: 1. To explore the factors of ethical decision-making of medical social workers in organ donation. Second, to help medical social workers to have a clearer understanding of the ethical dilemma and decision-making in organ donation. Third, through the research, let medical social workers understand the influence of personal life experience and values on organ donation. The purpose of this study was to investigate the perceived ethical issues of medical social workers in the social work of organ donation, as well as the decision-making process and factors. Medical social workers in the face of donors and family members, medical teams, laws and regulations, social culture and so on, which ethical issues are likely to cause difficulties for medical social workers? How can medical social workers make relevant decisions in the limited time? Based on this motivation, this study interviewed medical social workers engaged in organ donation in medical institutions through the method of data collection, so as to get a better understanding of this study. So that researchers have a deeper understanding of the social work of organ donation and the subject of this study. The study found that in the context of safeguarding the donor's right to informed consent, the protection of the privacy of the donor and recipient, the conflict between professional roles and team expectations, The conflict of personal values of medical social workers can easily lead to ethical dilemmas for medical social workers. (2) the main principles of ethical decision-making of medical social workers are to adhere to the principle of confidentiality, the legal provisions are fundamental, and the interests of the victims and their families are taken as the principle. In response to the findings of the above study, in the final part of the research recommendations, the author proposes that, in the personal aspect, the process of ethical decision-making action should be included in the record of individual cases, and at the same time, through continuous reflection on their attitude towards life, To examine the impact of individual values; to enhance the professional competence of new medical social workers in organ donation through a supervisory approach at the departmental level; and at the hospital level, to build consensus and trust inter-professional assessments through team meetings, At the same time, it establishes a good environment of institutional ethics, and advocates organ donation while ensuring respect for the privacy of donors and their families.
