面向養(yǎng)老院的健康管理服務(wù)設(shè)計(jì)研究

【摘要】：我國老年人數(shù)量不斷增多,老齡化程度不斷加深,老年人養(yǎng)老已經(jīng)成為社會(huì)關(guān)注的熱點(diǎn)問題。而機(jī)構(gòu)養(yǎng)老作為傳統(tǒng)家庭養(yǎng)老的重要補(bǔ)充,已經(jīng)成為越來越多老年人的選擇。入住養(yǎng)老院的老年人在某種程度上都存在一定的健康問題,大部分老人均患有慢性病。因此,本課題從服務(wù)設(shè)計(jì)的角度出發(fā),運(yùn)用服務(wù)設(shè)計(jì)的理論與方法,對(duì)養(yǎng)老院的健康管理服務(wù)現(xiàn)狀進(jìn)行分析,歸納現(xiàn)有健康管理服務(wù)存在的問題,并針對(duì)性的提出設(shè)計(jì)策略,以優(yōu)化健康管理服務(wù)流程,滿足老人多層次的健康護(hù)理需求。首先,在對(duì)兩家養(yǎng)老院的預(yù)調(diào)研的過程中,筆者總結(jié)了健康管理服務(wù)系統(tǒng)的利益相關(guān)人群,及不同護(hù)理級(jí)別老人所享受的健康管理服務(wù),其次對(duì)健康管理服務(wù)系統(tǒng)相關(guān)人群,包括老人、醫(yī)生、護(hù)士、老人子女和養(yǎng)老護(hù)理員的現(xiàn)狀進(jìn)行分析,了解制約養(yǎng)老院健康管理服務(wù)系統(tǒng)的客觀因素。對(duì)健康管理過程中的入院檔案建立與健康信息管理、健康監(jiān)測與評(píng)估、健康指導(dǎo)與干預(yù)、突發(fā)緊急狀況處理等接觸點(diǎn)進(jìn)行分析,了解健康管理的現(xiàn)狀,通過對(duì)三家養(yǎng)老機(jī)構(gòu)在健康監(jiān)測、健康評(píng)估和健康干預(yù)的內(nèi)容進(jìn)行比對(duì),最終得出了優(yōu)化信息反饋和完善健康信息管理兩個(gè)設(shè)計(jì)切入方向。其次,在后續(xù)的深入調(diào)研中設(shè)計(jì)訪談問題,并和養(yǎng)老院中的老人、醫(yī)生、護(hù)士、養(yǎng)老護(hù)理員和老人子女等相關(guān)人員進(jìn)行深入訪談,結(jié)合深入訪談、體驗(yàn)地圖等方法對(duì)養(yǎng)老院現(xiàn)有的健康管理服務(wù)中的問題點(diǎn)進(jìn)行挖掘,總結(jié)出健康管理服務(wù)系統(tǒng)中的具體需求點(diǎn),使用卡片分類的方法對(duì)具體的需求點(diǎn)進(jìn)行歸納,總結(jié)出現(xiàn)有的健康管理服務(wù)存在的四大問題:⒈服務(wù)種類單一⒉智能化應(yīng)用率低⒊信息傳遞流程長,信息滯后⒋信息的完整度與連續(xù)性差。然后用戶旅程方法對(duì)設(shè)計(jì)機(jī)會(huì)點(diǎn)進(jìn)行總結(jié),總結(jié)出設(shè)計(jì)可介入的問題點(diǎn)。進(jìn)而,針對(duì)以上問題,提出了⒈改變服務(wù)模式,增加健康管理服務(wù)種類⒉補(bǔ)充養(yǎng)老院的服務(wù)功能⒊合理高度地技術(shù)介入4.易用的信息傳遞系統(tǒng)的設(shè)計(jì)策略,滿足老人多層次的健康護(hù)理需求,保證健康管理服務(wù)提供的及時(shí)性,以及實(shí)現(xiàn)健康信息的集中管理與信息共享。最后,筆者提供的健康管理服務(wù)設(shè)計(jì)策略,使得老人、醫(yī)生、護(hù)士和養(yǎng)老護(hù)理員均能從中獲益。通過重塑服務(wù)形態(tài),可以滿足老人多層次的健康護(hù)理需求;并行的監(jiān)測機(jī)制,不但可以提高自理老人的自我健康管理參與性,而且可以減輕醫(yī)護(hù)人員、養(yǎng)老護(hù)理員的工作負(fù)擔(dān);異常信息反饋機(jī)制縮短了信息傳遞流程,保證服務(wù)提供的及時(shí)性。健康管理系統(tǒng)則保證信息的長久性、連續(xù)性和共享性,利用程序的自動(dòng)化評(píng)估,實(shí)現(xiàn)老人健康危險(xiǎn)的預(yù)警提醒。面向養(yǎng)老院的健康管理服務(wù)設(shè)計(jì)策略的提出,對(duì)養(yǎng)老院老人健康管理的研究者具有一定的啟示與借鑒作用,并可促進(jìn)更多研究者對(duì)養(yǎng)老院的健康服務(wù)系統(tǒng)進(jìn)行深入研究和改善,為老人提供更多樣、更全面的健康管理服務(wù)。
