【摘要】：目的:探討凝膠卡法鑒定ABO血型正反定型不一致的原因及其疾病分布的影響因素,并提出解決方法。方法:收集凝膠卡初檢血型正反定型不一致血液標(biāo)本240例,送血型參比實(shí)驗(yàn)室做經(jīng)典試管法及唾液血型物質(zhì)等血清學(xué)實(shí)驗(yàn),在血清學(xué)方法無(wú)法確定時(shí)進(jìn)行基因分型方法試驗(yàn)。結(jié)果:在ABO血型正反定型初檢不一致標(biāo)本240例中,假凝集采用試管法后正反定型一致107例(44.58%),不一致的133例(55.42%),其中冷凝集素35例(14.58%)、AB抗原性減弱22例(9.17%)、ABO亞型16例(6.67%)、直抗陽(yáng)性12例(5.00%)、抗體缺失或減弱11例(4.58%)、因藥物及蛋白引起假凝集11例(4.58%)、唾液血型物質(zhì)11例(4.58%)、ABO血型以外的同種抗體8例(3.33%)、異基因骨髓移植7例(2.92%);疾病分布情況為:血液病(16.83%)、腫瘤(11.88%)、心肺疾病(11.39%),經(jīng)卡方適度檢驗(yàn)分析,疾病分布差異顯著。結(jié)論:ABO血型正反定型不一致有多種因素影響,其中凝膠卡血型鑒定假陽(yáng)性較多;在血液病、腫瘤、心肺疾病等方面高發(fā),應(yīng)予以足夠的重視。
[Abstract]:Objective: to investigate the causes of inconsistency of positive and negative typing of ABO blood group by gel card method and the influencing factors of disease distribution, and to put forward some solutions. Methods: 240 blood samples with inconsistent blood group positive and negative typing were collected. The classical test tube method and saliva blood group substance serological test were carried out in the blood group reference laboratory. When the serological method could not be determined, the genotyping test was carried out. Results: among 240 cases of inconsistency of positive and negative typing of ABO blood group, 107 cases (44.58%) were positive and negative, 133 cases (55.42%) were inconsistent, including 35 cases (14.58%) of), AB antigenicity weakened (9.17%), ABO subgroup 16 cases (6.67%), 12 cases (5.00%) of direct antibody positive, 11 cases (4.58%) of antibody deletion or weakening. False aggregation caused by drugs and proteins was found in 11 cases (4.58%), saliva blood group substances in 11 cases (4.58%) other than), ABO blood group in 8 cases (3.33%), allogenic bone marrow transplantation in 7 cases (2.92%). The distribution of diseases was as follows: hematological diseases (16.83%), tumors (11.88%) and cardiopulmonary diseases (11.39%). The distribution of diseases was significantly different by chi-square moderate test. Conclusion: there are many factors affecting the inconsistency of positive and negative typing of ABO blood group, among which the false positive rate of gel card blood group identification is more, and the high incidence of blood disease, tumor, cardiopulmonary disease and so on should be paid enough attention to.
