新生兒臍部定植菌群特征分析

【摘要】:目的研究不同時期新生兒臍部細菌定植的分布特點,探討臍部細菌定植與侵入性感染發(fā)生的可能聯(lián)系。方法采用橫斷面整群抽樣調查研究方法,對143例患兒的臍部殘端拭子和血培養(yǎng)結果及臨床資料進行回顧性分析。結果 143例患兒的臍部殘端拭子培養(yǎng)有83.9%可見細菌生長,臍部細菌的定植率隨著出生后時間的延長逐漸增高,出生后24 h內的定植率與24 h后的定植率有明顯差異(P0.05)。兩組常見的定植菌為凝固酶陰性葡萄球菌、大腸埃希菌和金黃色葡萄球菌,早期新生兒組(≤7 d)更易發(fā)生腸球菌、肺炎克雷伯菌、草綠色鏈球菌和無乳鏈球菌的定植。76例患兒同時進行了臍部拭子和血培養(yǎng),發(fā)生血行侵入性感染的病例均來自早期新生兒組。結論新生兒臍部細菌定植大多在出生后24 h內完成。早期新生兒和晚期新生兒臍部皮膚優(yōu)勢定植細菌差異不大,但早期新生兒部分次優(yōu)勢菌定植可能導致侵入致病。早期新生兒臍部細菌定植發(fā)生血行侵入性感染機會大于晚期新生兒,無乳鏈球菌等特殊菌群血行感染率較高。
[Abstract]:Objective to study the distribution of neonatal umbilical cord bacterial colonization in different periods and to explore the possible relationship between umbilical cord bacterial colonization and invasive infection. Methods A total of 143 cases of umbilical cord swabs, blood culture and clinical data were retrospectively analyzed by cross-sectional cluster sampling. Results the bacterial growth was observed in 83.9% of the 143 cases of umbilical stump swab culture, and the colonization rate of the umbilical cord bacteria gradually increased with the prolongation of postnatal time. The colonization rate within 24 hours after birth was significantly different from that after 24 hours (P0.05).
