【摘要】：目的探討醫(yī)護康一體化延續(xù)管理模式在預防老年臥床患者出院后家居壓力性損傷中的應用效果。方法將120例老年臥床患者按出院時間順序分為實驗組和對照組,分別由醫(yī)護康組成的外展小組于出院前兩天評估患者并制訂家庭康復計劃。實驗組按照醫(yī)護康一體化延續(xù)管理模式,由醫(yī)護康組成的外展小組對出院后老年臥床患者進行壓力性損傷高危因素評估、制訂針對性預防壓力性損傷康復計劃、群組管理、遠程護理聯(lián)合家庭訪視,適時跟進措施落實并進行效果檢查。對照組給予常規(guī)出院宣教及出院后的電話隨訪。采用壓力性損傷管理和生存質量量表,比較兩組出院時、出院后6個月、出院后12個月的生活質量及1年的壓力性損傷發(fā)生率。結果實驗組壓力性損傷發(fā)生率較對照組低,差異有統(tǒng)計學意義(P0.05);出院后6個月、12個月生活質量的生理領域、心理領域得分均高于對照組,差異有統(tǒng)計學意義(P0.05);兩組環(huán)境領域、社會領域得分比較無統(tǒng)計學意義(P0.05)。結論醫(yī)護康一體化延續(xù)管理可有效降低老年臥床患者居家壓力性損傷發(fā)生率,提高其生活質量。
[Abstract]:Objective to explore the effect of continuous management mode of health care in prevention of stress injury in elderly bedridden patients after discharge from hospital. Methods 120 elderly bedridden patients were divided into experimental group and control group according to the order of discharge time. In the experimental group, according to the continuous management model of integrated health care, the outreaching team composed of health care and health care was used to assess the high risk factors of stress injury in elderly bedridden patients after discharge, and to formulate the rehabilitation plan for prevention of stress injury and group management. Combined home visit, timely follow-up measures and effect check. The control group was given routine discharge education and telephone follow-up after discharge. Stress injury management and quality of life scale were used to compare the quality of life (QOL) and the incidence rate of 1 year stress injury between the two groups at discharge, 6 months after discharge and 12 months after discharge. Results the incidence of stress injury in the experimental group was lower than that in the control group (P0.05). Six months after discharge, 12 months after discharge, the scores of physical and psychological areas were higher than those of the control group, the difference was statistically significant (P0.05); there was no significant difference between the two groups in the environmental field and social field score (P0.05). Conclusion the continuous management of health care can effectively reduce the incidence of family stress injury and improve the quality of life of elderly patients with bed rest.
