ICU護(hù)士預(yù)防呼吸機(jī)相關(guān)性肺炎護(hù)理的知信行水平及影響因素研究

【摘要】：[目的]調(diào)查了解重癥監(jiān)護(hù)病房(Intensive Care Unit,ICU)護(hù)士對(duì)預(yù)防呼吸機(jī)相關(guān)性肺炎(Ventilator-AssociatedPneumonia,VAP)護(hù)理的知識(shí)、態(tài)度/信念、行為現(xiàn)狀,探索ICU護(hù)士實(shí)施預(yù)防VAP護(hù)理的影響因素,以提高ICU護(hù)士 VAP預(yù)防護(hù)理的依從性,為增強(qiáng)ICU護(hù)士在預(yù)防VAP方面的繼續(xù)教育及培訓(xùn)提供參考性建議。[方法]采取方便抽樣的方法,于2016年9月至10月對(duì)江蘇省南京市三級(jí)甲等醫(yī)院的ICU護(hù)士進(jìn)行問卷調(diào)查。問卷由ICU護(hù)士的一般人口學(xué)資料調(diào)查表、ICU護(hù)士預(yù)防VAP護(hù)理的知信行水平調(diào)查表兩部分組成。分別使用Epi Data3.1和SPSS22.0統(tǒng)計(jì)軟件對(duì)數(shù)據(jù)進(jìn)行錄入與分析。對(duì)一般資料采用中位數(shù)、平均數(shù)、標(biāo)準(zhǔn)差、構(gòu)成比等進(jìn)行統(tǒng)計(jì)描述;對(duì)ICU護(hù)士預(yù)防VAP護(hù)理知識(shí)、態(tài)度/信念和行為的影響因素使用單因素分析、t檢驗(yàn)、多元逐步回歸分析、χ2檢驗(yàn)、Pearson相關(guān)分析等統(tǒng)計(jì)學(xué)方法進(jìn)行分析。[結(jié)果]1.自制的《ICU護(hù)士預(yù)防呼吸機(jī)相關(guān)性肺炎護(hù)理的知信行水平調(diào)查表》具有較好的信度和效度。2.ICU護(hù)士 VAP預(yù)防護(hù)理知識(shí)滿分為18分,總均分為10.36±3.20分;態(tài)度/信念得分滿分為35分,總均分為23.76±6.22分;行為得分滿分為55分,總均分為36.58±8.90分。3.ICU護(hù)士 VAP預(yù)防護(hù)理的知識(shí)和態(tài)度/信念之間、態(tài)度/信念和行為之間、知識(shí)和行為之間均具有顯著相關(guān)性(r=0.472,P0.001;r=0.763,P0.001;r=0.503,P0.001),知識(shí)、態(tài)度/信念、行為三者之間互成正相關(guān)。4.單因素分析結(jié)果表明:一般情況中的年齡、有無專科資質(zhì)、ICU工作年限、職稱、婚姻狀況等因素對(duì)ICU護(hù)士關(guān)于VAP預(yù)防護(hù)理的知識(shí)掌握情況影響具有統(tǒng)計(jì)學(xué)意義(P0.05);一般情況中的年齡、有無�？瀑Y質(zhì)、能級(jí)、ICU工作年限、工作年限、職稱、第一學(xué)歷、婚姻狀況等因素對(duì)ICU護(hù)士關(guān)于VAP預(yù)防護(hù)理的態(tài)度/信念影響具有統(tǒng)計(jì)學(xué)意義(P0.05);一般情況中的年齡、有無�？瀑Y質(zhì)、能級(jí)、ICU工作年限、工作年限、職稱、婚姻狀況等因素對(duì)ICU護(hù)士關(guān)于VAP預(yù)防護(hù)理的行為依從性影響具有統(tǒng)計(jì)學(xué)意義(P0.05)。5.多元線性回歸分析結(jié)果表明:ICU工作年限進(jìn)入知識(shí)回歸方程,ICU護(hù)士在ICU的工作年限對(duì)知識(shí)掌握情況有影響;婚姻狀況和ICU工作年限進(jìn)入態(tài)度/信念回歸方程,ICU護(hù)士的婚姻狀況和ICU工作年限對(duì)態(tài)度/信念有影響;ICU工作年限進(jìn)入行為回歸方程,ICU護(hù)士在ICU的工作年限對(duì)行為依從性有影響。[結(jié)論]1.通過本次調(diào)查發(fā)現(xiàn),ICU護(hù)士對(duì)預(yù)防VAP護(hù)理知識(shí)欠缺,態(tài)度/信念和行為仍需進(jìn)一步鞏固和提高。2.影響ICU護(hù)士預(yù)防VAP護(hù)理知識(shí)的主要因素是ICU工作年限;態(tài)度/信念方面的主要影響因素是婚姻狀況和ICU工作年限;行為方面的主要影響因素是ICU工作年限。3.應(yīng)加強(qiáng)ICU護(hù)士對(duì)預(yù)防VAP護(hù)理知識(shí)的培訓(xùn)和學(xué)習(xí),鞏固其科學(xué)規(guī)范執(zhí)行指南的態(tài)度,強(qiáng)化實(shí)施VAP預(yù)防護(hù)理的行為。4.ICU護(hù)士預(yù)防VAP護(hù)理知識(shí)與態(tài)度/信念之間存在顯著相關(guān)性、態(tài)度/信念與行為之間存在顯著相關(guān)性、知識(shí)與行為之間存在顯著相關(guān)性。

Attitude / belief and behavior are positively related to each other. The results of univariate analysis showed that: in general, the age, the number of years of working in ICU, the professional title, The influence of marital status on the knowledge of VAP prevention and nursing of ICU nurses was statistically significant (P0.05). The influence of marital status on the attitude / belief of ICU nurses on VAP preventive nursing was statistically significant (P0.05). Marital status and other factors on ICU nurses about VAP preventive nursing behavior compliance has statistical significance (P0.05). 5. The results of multiple linear regression analysis showed that the number of years worked in ICU had an effect on the knowledge acquisition. Relationship between marital status and ICU working years entering attitude / belief regression equation. The marital status of nurses and the working years of ICU have influence on attitude / belief. The regression equation of ICU working years into behavior regression equation suggests that the working years of ICU nurses in ICU have an effect on behavior compliance. [conclusion] 1. Through this investigation, we found that nurses in ICU lack knowledge of VAP, and attitude / belief and behavior still need to be further strengthened and improved. 2. The main influencing factors of ICU nurses' knowledge of preventive VAP were the working years of ICU, the main factors of attitude / belief were marital status and ICU's working years, and the main influencing factors of behavior were ICU's working years. ICU nurses should strengthen their training and study on preventive VAP nursing knowledge, consolidate their attitude towards implementing scientific and normative guidelines, and strengthen the behavior of implementing VAP preventive nursing. 4. There is a significant correlation between VAP nurses' knowledge of preventive VAP nursing and their attitudes / beliefs. There was significant correlation between attitude / belief and behavior, and between knowledge and behavior.
