2016年安徽省滁州市第一人民醫(yī)院臨床分離菌耐藥性監(jiān)測(cè)

【摘要】:目的了解2016年滁州市第一人民醫(yī)院常見臨床分離菌的分布及耐藥情況。方法各類臨床分離菌采用MicroScan WalkAway 96 PLUS進(jìn)行鑒定和藥敏試驗(yàn),采用WHONET5.6軟件對(duì)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析。結(jié)果 2016年共分離病原菌1 441株,其中革蘭陽性菌409株,占28.4%;革蘭陰性菌1 032株,占71.6%。葡萄球菌中耐甲氧西林金黃色葡萄球菌(MRSA)和凝固酶陰性葡萄球菌(MRCNS)的檢出率分別為37.5%和83.0%。葡萄球菌中未見對(duì)萬古霉素、達(dá)托霉素和利奈唑胺耐藥的菌株。腸球菌中屎腸球菌對(duì)達(dá)托霉素、萬古霉素和利奈唑胺全敏感,檢出1株耐利奈唑胺糞腸球菌,檢出率為2.6%。肺炎鏈球菌主要來源于兒科,對(duì)青霉素全敏感。大腸埃希菌、克雷伯菌屬(肺炎克雷伯菌和產(chǎn)酸克雷伯菌)和奇異變形桿菌產(chǎn)ESBL的檢出率分別為49.1%、24.2%和62.5%,產(chǎn)ESBL株對(duì)監(jiān)測(cè)藥物的耐藥率均高于非產(chǎn)ESBL株。銅綠假單胞菌對(duì)亞胺培南和美羅培南的耐藥率均為10.4%。不動(dòng)桿菌屬(鮑曼不動(dòng)桿菌占95.0%)對(duì)亞胺培南和美羅培南的耐藥率分別為79.0%和78.2%。結(jié)論 2016年該醫(yī)院分離的革蘭陰性桿菌耐藥情況嚴(yán)重,肺炎克雷伯菌和鮑曼不動(dòng)桿菌對(duì)碳青霉烯類抗菌藥物的耐藥率呈上升趨勢(shì),應(yīng)引起重視。
[Abstract]:Objective to investigate the distribution and drug resistance of common clinical isolates in the first people's Hospital of Chuzhou City in 2016. Methods all kinds of clinical isolates were identified by MicroScan WalkAway 96 PLUS and drug sensitivity test. The data were statistically analyzed by WHONET5.6 software. Results A total of 1 441 strains of pathogenic bacteria were isolated in 2016, of which 409 were Gram positive bacteria (28. 4%), 1 032 were Gram-negative bacteria (71. 6%). The detectable rates of methicillin-resistant Staphylococcus aureus (MRSA) and coagulase-negative staphylococcus (MRCNS) were 37.5% and 83.0%, respectively. No vancomycin, daptomycin and linazolamide resistant strains were found in Staphylococcus. Enterococcus faecium was completely sensitive to daptomycin, vancomycin and linazolamide. A strain of Enterococcus faecalis resistant to renazolamine was detected, and the detection rate was 2.6%. Streptococcus pneumoniae mainly comes from paediatrics and is completely sensitive to penicillin. The positive rates of ESBL production of Escherichia coli, Klebsiella pneumoniae and Proteus mirabilis were 49.2% and 62.5%, respectively. The drug resistance rate of ESBL producing strains was higher than that of non-producing ESBL strains. The resistance rates of Pseudomonas aeruginosa to imipenem and meropenem were 10.4%. The resistance rates of Acinetobacter (Acinetobacter baumannii) to imipenem and meropenem were 79.0% and 78.2%, respectively. Conclusion the drug resistance of Gram-negative bacilli isolated from the hospital in 2016 is serious. The resistance rate of Klebsiella pneumoniae and Acinetobacter baumannii to carbapenem antibiotics is on the rise and should be paid more attention.
