【摘要】：目的提高慢性腎臟病患者的自我管理能力。方法運用專案改善方法分析慢性腎臟病患者自我管理能力差的原因,制訂隨訪管理及健康教育標準化方案,分析84例門診慢性腎臟病3~4期患者實施改善方案干預后自我管理能力水平、隨訪、服藥和飲食依從性及各生理指標達標率的變化。結(jié)果干預后,患者的自我管理能力各維度得分和隨訪、服藥和飲食依從性均顯著提高(P0.05),收縮壓和血尿酸達標率較干預前改善顯著,差異有統(tǒng)計學意義(P0.001)。結(jié)論實施護理專案改善可提高門診慢性腎臟病3~4期患者自我管理能力和隨訪、服藥及飲食依從性,改善其生理指標。
[Abstract]:Objective to improve the self-management ability of patients with chronic kidney disease. Methods the causes of poor self-management ability of patients with chronic kidney disease were analyzed by using the method of project improvement, and the standardized plan of follow-up management and health education was established. The self-management ability of 84 outpatient patients with chronic kidney disease at stage 3 and 4 were analyzed. The changes of self-management ability, follow up, compliance with medication and diet, and the rate of reaching the standard of physiological indexes were analyzed in 84 patients with chronic kidney disease in the outpatient department. Results after intervention, the scores of each dimension of self-management ability, follow-up, medication and diet compliance were significantly improved (P0.05), systolic blood pressure and blood uric acid reached the standard rate improved significantly (P0.001). Conclusion the improvement of nursing project can improve the ability of self-management, follow up, drug taking and diet compliance, and improve the physiological indexes of the patients with chronic kidney disease.
