【摘要】：目的探討利用抗菌藥物的PK/PD特點(diǎn)及藥物濃度監(jiān)測(cè)結(jié)果指導(dǎo)多藥耐藥菌感染的重癥患者的個(gè)體化抗感染治療策略及連續(xù)性腎臟替代治療(CRRT)治療時(shí)藥物劑量的調(diào)整,提高重癥感染患者救治成功率。方法對(duì)2013年西安交通大學(xué)第一附屬醫(yī)院重癥醫(yī)學(xué)科收治的1例多藥耐藥菌感染的重癥患者的臨床、影像學(xué)及病原學(xué)資料,抗菌藥物的PK/PD特點(diǎn)及藥物濃度監(jiān)測(cè)結(jié)果,以及CRRT治療時(shí)的藥物劑量調(diào)整進(jìn)行分析總結(jié)。結(jié)果患者病情危重,治療過(guò)程中痰培養(yǎng)為泛耐藥鮑氏不動(dòng)桿菌,血培養(yǎng)曾出現(xiàn)多藥耐藥的肺炎克雷伯菌肺炎亞種及近平滑假絲酵母菌、尿培養(yǎng)曾出現(xiàn)多藥耐藥的肺炎克雷伯菌肺炎亞種及多藥耐藥的屎腸球菌。治療中我們根據(jù)藥敏結(jié)果結(jié)合患者臟器功能選擇抗菌藥物,使用美羅培南對(duì)耐藥菌進(jìn)行治療時(shí),采用兩步法給藥,根據(jù)抗菌藥物的PK/PD特點(diǎn)優(yōu)化抗菌藥物使用,分析CRRT治療對(duì)藥物濃度的影響,對(duì)治療藥物進(jìn)行濃度監(jiān)測(cè),根據(jù)監(jiān)測(cè)結(jié)果對(duì)抗菌藥物進(jìn)行個(gè)體化調(diào)整,以發(fā)揮抗菌藥物的最大療效,最終患者好轉(zhuǎn)出院。結(jié)論對(duì)于多藥耐藥菌混合感染的重癥患者,利用抗菌藥物的PK/PD特點(diǎn)優(yōu)化抗菌藥物治療。根據(jù)治療中藥物濃度監(jiān)測(cè)結(jié)果調(diào)整抗菌藥物劑量,實(shí)現(xiàn)精準(zhǔn)化、個(gè)體化治療,提高重癥患者的救治成功率。
[Abstract]:Objective to explore the individualized anti infection treatment strategy of the severe patients with multidrug-resistant bacteria infection and the adjustment of drug dosage during the treatment of continuous renal replacement therapy (CRRT) by using the PK/PD characteristics of antimicrobial agents and the monitoring results of drug concentration, and to improve the successful rate of treatment for patients with severe infection. Methods for the First Affiliated of Xi'an Jiao Tong University in 2013. The clinical, imaging and pathogenic data of 1 cases of multidrug resistant bacteria, the PK/PD characteristics of antimicrobial agents, the results of drug concentration monitoring, and the drug dose adjustment during CRRT treatment were analyzed and summarized in the hospital. In the blood culture, there were multidrug-resistant Klebsiella pneumoniae subspecies and nearly smooth Candida. In urine culture, multidrug-resistant Klebsiella pneumoniae pneumonia subspecies and multidrug-resistant Enterococcus faecium were found. In the course of treatment, two steps were used to optimize the use of antibacterial drugs according to the PK/PD characteristics of antibiotics, to analyze the effect of CRRT therapy on the drug concentration, to monitor the concentration of the drugs, and to adjust the antimicrobial agents individually according to the monitoring results, so as to give full play to the efficacy of the antibacterial drugs and ultimately improve the discharge of the hospital. The severe patients with drug resistant bacteria mixed infection use the PK/PD characteristics of antibacterials to optimize the treatment of antibiotics. Adjust the dose of antibiotics according to the results of drug concentration monitoring in the treatment, achieve precision, individualized treatment, and improve the success rate of treatment for severe patients.
