血培養(yǎng)病原菌分布及耐藥分析

【摘要】:目的:分析包頭醫(yī)學(xué)院第二附屬醫(yī)院2015年1月-2016年11月血培養(yǎng)中病原菌分布及其對(duì)抗菌藥物的敏感情況,為臨床合理使用抗菌藥物提供依據(jù)。方法:采用美國(guó)BD公司的BACTEC 9050全自動(dòng)血培養(yǎng)儀,對(duì)血培養(yǎng)瓶進(jìn)行連續(xù)培養(yǎng)監(jiān)測(cè),BD公司生產(chǎn)的Phoenix 100系統(tǒng)全自動(dòng)微生物分析系統(tǒng)對(duì)血標(biāo)本中分離的病原菌進(jìn)行鑒定和藥物敏感試驗(yàn),利用WHONET 5.6軟件來(lái)對(duì)藥敏結(jié)果進(jìn)行分析。結(jié)果:1307份血培養(yǎng)標(biāo)本中,病原菌的分離數(shù)量共182株,陽(yáng)性檢出率為13.9%,其中革蘭陰性菌120株,占65.9%;革蘭陽(yáng)性菌61株,占33.5%;真菌1株,占0.6%。對(duì)大腸埃希菌、克雷伯菌屬、葡萄球菌屬、腸球菌屬進(jìn)行藥敏結(jié)果分析,葡萄球菌屬對(duì)萬(wàn)古霉素、利奈唑胺100%敏感,大腸埃希菌以及克雷伯菌屬對(duì)亞胺培南和美羅培南100%敏感,其他抗菌藥物則有不同程度的耐藥。結(jié)論:目前本院血培養(yǎng)分離出的病原菌主要以革蘭陰性菌為主,菌種多樣化,且呈現(xiàn)較高的耐藥率,提示臨床醫(yī)生應(yīng)重視血培養(yǎng),以便合理應(yīng)用抗菌藥物。
[Abstract]:Objective: to analyze the distribution of pathogenic bacteria and its susceptibility to antibiotics in the second affiliated Hospital of Baotou Medical College from January 2015 to November 2016 so as to provide evidence for rational use of antibiotics in clinical practice. Methods: the BACTEC 9050 automatic blood culture instrument of BD Company was used to monitor the continuous culture of the blood culture bottle. The Phoenix 100 system automatic microbiological analysis system produced by BD Company was used to identify the pathogenic bacteria isolated from the blood samples and to test their drug sensitivity. The results of drug sensitivity were analyzed by WHONET 5.6 software. Results 182 strains of pathogenic bacteria were isolated from 1 07 blood samples, and the positive rate was 13. 9%, of which 120 were Gram-negative bacteria (65. 9%), 61 were Gram-positive bacteria (33. 5%) and 1 was fungi (0. 6%). The susceptibility of Escherichia coli, Klebsiella, Staphylococcus and Enterococcus to vancomycin and linazolamine was 100%, and Escherichia coli and Klebsiella were 100% sensitive to imipenem and meropenem. Other antimicrobial agents have varying degrees of resistance. Conclusion: the main pathogenic bacteria isolated from blood culture in our hospital at present are Gram-negative bacteria, variety of bacteria species and high drug resistance rate, suggesting that clinicians should pay attention to blood culture in order to use antibiotics rationally.
