【摘要】：目的:評(píng)價(jià)腺苷脫氨酶(ADA)、結(jié)核抗體(TB-Ab)以及結(jié)核分枝桿菌DNA(TB-DNA)聯(lián)合檢測(cè)在結(jié)核性胸膜炎的診斷價(jià)值。方法:分別檢測(cè)病例組(82例結(jié)核性胸膜炎患者)和對(duì)照組(80例非結(jié)核性胸膜炎患者)胸水中ADA、TB-Ab和TB-DNA指標(biāo)陽(yáng)性率,對(duì)單一和聯(lián)合檢測(cè)結(jié)果進(jìn)行評(píng)價(jià)。結(jié)果:病例組患者ADA、TB-Ab以及TBDNA陽(yáng)性率均高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);ADA、TB-Ab和TB-DNA聯(lián)合檢測(cè)的診斷價(jià)值最高,靈敏度為87.80%,特異度為83.75%,約登指數(shù)為0.716,陽(yáng)性預(yù)測(cè)值為84.71%,陰性預(yù)測(cè)值為87.01%。結(jié)論:ADA、TB-Ab和TB-DNA在結(jié)核性胸膜炎患者中的陽(yáng)性率較高,并且三指標(biāo)聯(lián)合檢測(cè)對(duì)結(jié)核性胸膜炎具有較高的診斷價(jià)值,有助于早期診斷。
[Abstract]:Objective: to evaluate the diagnostic value of adenosine deaminase ADAA, TB antibody TB-Aband Mycobacterium tuberculosis DNA TB-DNA in the diagnosis of tuberculous pleurisy. Methods: the positive rates of ADA-TB-Ab and TB-DNA in pleural effusion of 82 patients with tuberculous pleurisy and 80 patients with non-tuberculous pleurisy in the control group were detected, and the results of single and combined detection were evaluated. Results: the positive rates of ADA-TB-Ab and TBDNA in the case group were higher than those in the control group. The difference was statistically significant (P 0.05). The diagnostic value of TB-Ab combined with TB-DNA was the highest, the sensitivity was 87.80, the specificity was 83.755.The Yorden index was 0.716, the positive predictive value was 84.71, and the negative predictive value was 87.01. Conclusion the positive rate of TB-Ab and TB-DNA in tuberculous pleurisy is higher than that of TB-Ab in tuberculous pleurisy, and the combined detection of three indexes has high diagnostic value for tuberculous pleurisy, which is helpful to the early diagnosis of tuberculous pleurisy.
