【摘要】：目的探討降鈣素原與中性粒細胞淋巴細胞計數(shù)比對心血管疾病住院患者病原菌感染的早期診斷價值。方法選擇2014年1月-2016年10月醫(yī)院心內(nèi)科及干部病房住院的652例可能有病原菌感染的老年患者作為研究對象,按照病原菌培養(yǎng)結(jié)果分為感染組476例和非感染組176例,分析降鈣素原(PCT),白細胞計數(shù)(WBC),中性粒細胞計數(shù)(NE)和NLR四個指標針對感染的診斷價值。結(jié)果 652名患者的病原菌培養(yǎng)結(jié)果中,陽性476例占73.01%;陰性176例,占26.99%,感染部位以呼吸道和泌尿系統(tǒng)為主,共分離病原菌476株,其中革蘭陽性菌350株占73.53%,革蘭陽性菌126株占26.47%,未出現(xiàn)兩種以上病原菌的合并感染;感染組WBC,NE,PCT和NLR分別為(14.13±4.32)×109/L,(0.73±0.13)×109/L,(1.25±0.42)ng/ml,(2.59±0.75),顯著高于非感染組(10.52±3.95)×109/L,(0.58±0.14)×109/L,(0.40±0.13)ng/ml,(1.42±0.62),PCT的靈敏度和特異度最高,在PCT和NLR兩個指標并聯(lián)使用的情況下,靈敏度進一步增高。結(jié)論心血管疾病老年患者感染以呼吸與泌尿系統(tǒng)為主,病原菌分布與常規(guī)院內(nèi)感染分布類似,在PCT和NLR兩個指標并聯(lián)使用的情況下,靈敏度增高,適合對心血管疾病老年患者的病原菌感染進行早期診斷。
[Abstract]:Objective to investigate the early diagnostic value of procalcitonin / neutrophil lymphocyte count ratio in the early diagnosis of pathogenic bacteria infection in patients with cardiovascular disease.Methods from January 2014 to October 2016, 652 elderly patients with possible pathogenic bacteria infection were selected from cardiology department and cadre ward of hospital from January 2014 to October 2016.
