腦白質(zhì)疏松癥患者早期認(rèn)知功能損害的特點(diǎn)及彌散張量成像研究

【摘要】:目的:分析腦白質(zhì)疏松癥(LA)患者早期認(rèn)知功能損害的臨床特點(diǎn)和各感興趣區(qū)磁共振彌散張量成像的FA值變化,并探討LA患者白質(zhì)變化與認(rèn)知功能的相關(guān)性。方法:選擇2015年8月至2016年10月在山西省人民醫(yī)院收治的慢性缺血性LA患者38例和20例健康患者行常規(guī)MRI及DTI檢查,采用神經(jīng)心理學(xué)量表評價(jià)不同認(rèn)知域損害特點(diǎn),測量腦內(nèi)不同感興趣區(qū)的部分各向異性分?jǐn)?shù)(FA值),并作相關(guān)性分析。結(jié)果:1.與對照組相比,LA患者早期認(rèn)知功能損害特點(diǎn),表現(xiàn)為總體認(rèn)知功能減退,執(zhí)行功能、記憶力、視空間結(jié)構(gòu)技能、信息處理能力顯著下降(P0.05),注意力無明顯下降(P0.05)。2.LA患者在雙側(cè)側(cè)腦室前角、左額上回、左額下回、雙側(cè)額葉眶回、右側(cè)顳葉深部白質(zhì)、右側(cè)扣帶回、胼胝體膝部FA值較正常對照組的FA值顯著下降,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。3.LA分級越嚴(yán)重,FA值越低,呈負(fù)相關(guān)(P0.05)。4.記憶力與左側(cè)側(cè)腦室前角、雙側(cè)額葉眶回、右顳葉深部白質(zhì)、右扣帶回、胼胝體膝部腦區(qū)呈正相關(guān),與左額下回呈負(fù)相關(guān)(P0.05);信息處理能力與右側(cè)側(cè)腦室前角呈正相關(guān),與右顳葉深部白質(zhì)區(qū)呈負(fù)相關(guān)(P0.05);執(zhí)行功能與左額葉眶回、右顳葉深部白質(zhì)、右扣帶回、胼胝體膝部等區(qū)FA值呈負(fù)相關(guān)(P0.05),視空間能力與左側(cè)額葉眶回、右側(cè)顳葉深部白質(zhì)、胼胝體膝部呈正相關(guān)(P0.05)。結(jié)論:LA患者出現(xiàn)早期認(rèn)知功能損害時(shí),部分腦區(qū)FA值會發(fā)生變化,且與認(rèn)知功能損害程度呈負(fù)相關(guān)。彌散張量成像有助于診斷腦白質(zhì)疏松癥患者早期認(rèn)知功能損害。
[Abstract]:Objective: to analyze the clinical features of early cognitive impairment in patients with leukoaraiosis and the changes of FA values of diffusive Zhang Liang imaging in different areas of interest. Methods: from August 2015 to October 2016, 38 patients with chronic ischemic LA and 20 healthy controls were examined by routine MRI and DTI. The neuropsychological scale was used to evaluate the characteristics of cognitive impairment in different cognitive domains, and the partial anisotropic fraction (FA) of different regions of interest in the brain was measured, and the correlation analysis was made. Results: 1. Compared with the control group, the early cognitive impairment characteristics of LA patients were analyzed. The results showed that the overall cognitive function, executive function, memory, visual spatial structure skills, information processing ability were significantly decreased (P 0.05), and attention was not significantly decreased in patients with LA in bilateral anterior ventricle horn, left superior frontal gyrus, left inferior frontal gyrus, bilateral frontal orbital gyrus, bilateral frontal lobe orbital gyrus, bilateral frontal lobe orbital gyrus, left superior frontal gyrus, left superior frontal gyrus, bilateral frontal lobe orbital gyrus. The FA value of the deep white matter of the right temporal lobe, the right cingulate gyrus and the genu of corpus callosum was significantly lower than that of the normal control group, and the difference was statistically significant (P 0.05). 3. The more serious the grade of LA was, the lower the FA value was, and the negative correlation was found between the memory and the anterior horn of the left lateral ventricle. Bilateral frontal lobe orbital gyrus, deep white matter of right temporal lobe, right cingulate gyrus, genu of corpus callosum were positively correlated, and negatively correlated with left inferior frontal gyrus (P0.05). There was a negative correlation between the executive function and the FA values of left frontal lobe orbital gyrus, right temporal lobe deep white matter, right cingulate gyrus and genu of corpus callosum. The visual spatial ability was negatively correlated with the left frontal orbital gyrus and the deep white matter of right temporal lobe. The genu of corpus callosum had a positive correlation with P0.05.Conclusion the FA value of some brain regions will change in the early stage of cognitive impairment in patients with% LA. Diffusion Zhang Liang imaging was helpful in the diagnosis of early cognitive impairment in patients with leukoaraiosis.
