【摘要】：目的探討超聲雙重造影(DCEU)對大腸腫瘤篩查的臨床應(yīng)用價值。方法 (1)受檢查者先行常規(guī)經(jīng)腹壁超聲檢查(CTU),然后清潔腸道,將胃腸聲學(xué)造影劑從肛門灌入腸道后,再進行大腸充盈型造影超聲檢查(IFCU)。(2)對懷疑大腸腫瘤者進行靜脈造影超聲檢查(ICEU)、纖維腸鏡及病理學(xué)檢查。(3)DCEU、纖維腸鏡、病理學(xué)檢查三者結(jié)果進行對比分析,經(jīng)統(tǒng)計學(xué)處理后,研究DCEU對大腸腫瘤篩查的臨床應(yīng)用價值。結(jié)果 (1)CTU及IFCU檢查266例,DCEU檢查45例;經(jīng)病理學(xué)檢查確診大腸腫瘤41例,CTU發(fā)現(xiàn)20例,DCEU發(fā)現(xiàn)39例,表明DCEU對大腸腫瘤的篩查更具檢出性(χ2=21.81,P=0.00)。(2)大腸腫瘤DCEU聲像圖各具特點。(3)DCEU對大腸腫瘤定位準確率較CTU明顯增高(χ2=28.15,P=0.00)。(4)對良、惡性大腸腫瘤定性符合率DCEU為88.89%、96.88%,纖維腸鏡為100%、93.75%,DCEU及纖維腸鏡對大腸腫瘤的檢出均是有效的(P0.05)。結(jié)論大腸雙重超聲造影檢查法為大腸腫瘤好發(fā)人群篩查提供有效可行的手段,具有重要的臨床推廣應(yīng)用價值。
[Abstract]:Objective to investigate the value of double contrast-enhanced ultrasonography (DCEU) for colorectal cancer screening in the clinical application. Methods (1) were examined by routine transabdominal ultrasonography (CTU), and then clean the intestinal tract, gastrointestinal tract contrast agent into the intestinal tract from the anus after irrigation, then E. filling type contrast sonography ((IFCU). 2) intravenous contrast enhanced ultrasound examination of suspected colorectal tumor (ICEU), fiber colonoscopy and pathological examination. (3) DCEU, colonoscopy, pathological examination of the three results were analyzed by statistical treatment, clinical application value of DCEU for colorectal cancer screening. Results (1) CTU and IFCU in 266 cases, 45 cases of DCEU examination; pathology confirmed 41 cases of colorectal cancer, 20 cases were found by CTU, 39 cases were found by DCEU, showed that DCEU screening for colorectal cancer is more detection of (x 2=21.81, P=0.00). (2) colorectal tumor DCEU sonographic characteristics. (3) DCEU colorectal neoplasms The positioning accuracy was significantly higher than that of CTU (2=28.15, P=0.00). (4) of benign and malignant colorectal tumors, qualitative coincidence rate of DCEU was 88.89%, 96.88%, 93.75%, 100% for colonoscopy, DCEU and colonoscopy detection of colorectal cancer are effective (P0.05). Conclusion ultrasound imaging method of double E. for colorectal tumor screening and provide a feasible and effective method, and has important clinical application value.
