【摘要】：目的分析實施綜合接觸隔離措施對耐碳青霉烯類肺炎克雷伯菌(CRKP)、鮑氏不動桿菌(CRAB)感染發(fā)生狀況的影響,為預防與控制耐碳青霉烯類細菌院內(nèi)傳播提供循證依據(jù)。方法采用回顧性調(diào)查,收集干預前醫(yī)院2013年1月1日-2014年12月31日肺炎克雷伯菌及鮑氏不動桿菌感染患者的臨床信息,2015年1月1日-2016年12月31日實施綜合接觸隔離措施,措施包括開具接觸隔離醫(yī)囑、標識、信息干預,手衛(wèi)生宣傳活動等,比較干預前后CRKP及CRAB的隔離率和感染率的變化。結(jié)果干預后CRKP隔離率從20.95%升至61.82%,CRAB從16.80%升至51.49%,差異有統(tǒng)計學意義(P0.05);手衛(wèi)生依從率從63.93%上升至83.86%,差異有統(tǒng)計學意義(P0.05);CRKP、CRAB的醫(yī)院感染率分別從0.51‰、0.68‰降至0.19‰、0.37‰,差異有統(tǒng)計學意義(P0.05);CRKP檢出率從13.15%下降至9.31%,差異有統(tǒng)計學意義(P0.05),CRAB檢出率從36.43%上升至40.27%,差異無統(tǒng)計學意義(P0.05)。結(jié)論開具接觸隔離醫(yī)囑、標識明確、信息干預、提高手衛(wèi)生依從率等綜合接觸隔離措施,能有效減少CRKP、CRAB醫(yī)院感染的發(fā)生。
[Abstract]:Objective to analyze the effect of comprehensive contact isolation on the incidence of Klebsiella carpopilli, Acinetobacter baumannii (Acinetobacter baumannii) and Klebsiella pneumoniae (Klebsiella baumannii) infection. To provide evidence-based basis for preventing and controlling the spread of carbapenem-resistant bacteria in hospital. Methods retrospective investigation was carried out. To collect clinical information of patients with Klebsiella pneumoniae and Acinetobacter baumannii infection in pre-intervention hospitals from January 1st 2013 to December 31st 2014. From January 1st 2015 to December 31st 2016, comprehensive contact isolation measures were carried out, including issuing contact isolation doctor's orders, marking, information intervention, hand hygiene publicity activities and so on. The isolation rate and infection rate of CRKP and CRAB were compared before and after intervention. Results the isolation rate of CRKP increased from 20.95% to 61.82% after intervention. CRAB increased from 16.80% to 51.49, and the difference was statistically significant (P 0.05). The compliance rate of hand hygiene increased from 63.93% to 83.86, and the difference was statistically significant (P 0.05). The nosocomial infection rate of CRKP- CRAB decreased from 0.51 鈥，
