【摘要】：[目的]調(diào)查廣東省各級醫(yī)院護(hù)士用藥錯誤現(xiàn)狀及其影響因素,為制定用藥錯誤的護(hù)理質(zhì)量改善指引提供依據(jù)。[方法]采用便利抽樣方法抽取廣東省一級、二級、三級醫(yī)院90家,采用自行設(shè)計的"護(hù)士用藥錯誤影響因素調(diào)查表"對臨床一線的1 528名護(hù)士進(jìn)行問卷調(diào)查。[結(jié)果]231名(15.12%)護(hù)士過去1年內(nèi)發(fā)生過用藥錯誤,共計441例次;排名前5位的分別為用藥時間錯誤、劑量錯誤、漏打漏發(fā)、病人錯誤、藥物錯誤;醫(yī)院級別、職稱、工作環(huán)境、醫(yī)院的發(fā)藥系統(tǒng)、使用掌上電腦(PDA)掃描核對技術(shù)、主動上報、懲罰方式、整改分析是護(hù)士用藥錯誤的主要影響因素,其中醫(yī)院的發(fā)藥系統(tǒng)、工作環(huán)境、醫(yī)院級別及職稱是主要影響因素;使用PDA掃描核對技術(shù)、建立主動上報、適當(dāng)?shù)膽土P及整改分析機(jī)制是護(hù)士用藥錯誤的保護(hù)性因素。[結(jié)論]護(hù)士發(fā)生用藥錯誤與多種因素有關(guān),醫(yī)院管理者應(yīng)針對性地制定用藥安全質(zhì)量改進(jìn)指引,降低護(hù)士用藥錯誤,保障病人用藥安全。
[Abstract]:[Objective] to investigate the status quo and influencing factors of drug use errors among nurses in hospitals at all levels in Guangdong Province, and to provide the basis for formulating guidelines for improving nursing quality. [Methods] the convenience sampling method was used to select 90 hospitals of first, second and third grade in Guangdong Province. The self-designed questionnaire of influencing factors of Drug use errors of Nurses was used to investigate 1 528 nurses in clinical front line. [Results] 231 nurses (15.12) had medication errors in the past year, a total of 441 times; The top 5 were time error, dosage error, missed occurrence, patient error and drug error. Hospital level, professional title, working environment, hospital drug delivery system, the use of PDA-scan check technology, active reporting, punishment, rectification and analysis are the main factors that affect the drug use errors of nurses. The distribution system, working environment, hospital grade and professional title are the main influencing factors. Using PDA scanning and checking technology, establishing active reporting, appropriate punishment and corrective analysis mechanism are the protective factors of nurses' medication errors. [Conclusion: the occurrence of drug errors among nurses is related to many factors. Hospital administrators should formulate guidelines to improve the safety and quality of drug use, reduce the errors of the use of drugs by nurses, and ensure the safety of drug use by patients.
