【摘要】:目的應(yīng)用實(shí)時(shí)三維超聲心動(dòng)圖(RT-3DE)探討肥胖和超重對(duì)左房(LA)容積和功能的影響。方法按體質(zhì)量指數(shù)(BMI)將120例健康體檢者分為三組:肥胖組、超重組和對(duì)照組,應(yīng)用RT-3DE檢測(cè)左房最大容積(LAVmax)、左房收縮前容積(LAVp)及左房最小容積(LAVmin),計(jì)算并比較各組間左房容積指數(shù)(LAVI)、LA總排空容積(LATEV)、LA總排空指數(shù)(LAEFt)、LA被動(dòng)排空容積(LAPEV)、LA被動(dòng)排空指數(shù)(LAEFp)、LA主動(dòng)排空容積(LAAEV)、LA主動(dòng)排空指數(shù)(LAEFa)。分析N末端腦利鈉肽前體(NT-proBNP)和左房容積及功能指標(biāo)的相關(guān)性。結(jié)果 (1)對(duì)照組、超重組及肥胖組的LA容積指標(biāo)(LAVmax、LAVp、LAVmin、LAVI)依次增大,儲(chǔ)存功能指標(biāo)(LATEV)和LA泵功能指標(biāo)(LAAEV、LAEFa)依次增大,而LA傳導(dǎo)功能指標(biāo)(LAEFp)則依次減小。(2)肥胖組NTproBNP水平稍高于超重組和對(duì)照組,差異無(wú)統(tǒng)計(jì)學(xué)意義。NT-proBNP與LAVI、LAVmax、LAVmin、LATEV及LAAEV具有一定相關(guān)性。結(jié)論肥胖及超重可在無(wú)心血管疾病癥狀前引起左房結(jié)構(gòu)和機(jī)械功能受損。RT-3DE可以靈敏地檢測(cè)出上述亞臨床改變。
[Abstract]:Objective to investigate the effect of obesity and overweight on the volume and function of left atrial (LA) by real time three-dimensional echocardiography (RT-3DE). Methods according to body mass index (BMI) and 120 healthy subjects were divided into three groups: the overweight and obesity group, control group, left atrial volume measured by RT-3DE maximum (LAVmax), left atrial systolic volume (LAVp) and minimum left atrial volume (LAVmin), left atrial volume index were calculated and compared between LA (LAVI), total emptying volume (LATEV), LA index (LAEFt), total emptying LA passive emptying volume (LAPEV), LA index (LAEFp), passive emptying LA active emptying volume (LAAEV), LA active emptying index (LAEFa). The correlation between the N terminal brain natriuretic peptide precursor (NT-proBNP), the left atrial volume and the functional indexes was analyzed. Results (1) the LA volume index (LAVmax, LAVp, LAVmin and LAVI) increased in control group, overweight group and obesity group. Storage function index (LATEV) and LA pump function index (LAAEV and LAEFa) increased in sequence, while LA conduction function index (LAEFp) decreased in turn. (2) the level of NTproBNP in the obese group was slightly higher than that in the super recombinant and the control group, with no statistical difference. NT-proBNP has a certain correlation with LAVI, LAVmax, LAVmin, LATEV and LAAEV. Conclusion obesity and overweight can cause damage to the left atrial structure and mechanical function before the symptoms of cardiovascular disease. RT-3DE can be used to detect these subclinical changes sensitively.
