【摘要】：隨著工業(yè)化和城鎮(zhèn)化的發(fā)展、人口老齡化進(jìn)程的加快和生活方式的改變,慢性病的患病率呈逐漸增長(zhǎng)的趨勢(shì)。日益增長(zhǎng)的慢性病人群的治療和護(hù)理帶來了極大的經(jīng)濟(jì)負(fù)擔(dān)和衛(wèi)生服務(wù)挑戰(zhàn)。對(duì)慢性病患者加強(qiáng)健康教育,提高患者的自我管理能力,能夠有效地控制慢性病病情,延緩并發(fā)癥的發(fā)生,從而減輕患者經(jīng)濟(jì)負(fù)擔(dān),提高醫(yī)療投入產(chǎn)出比,改善慢性病防治現(xiàn)狀。要做好慢性病自我管理,除了需要患者了解自身的疾病情況,還要求患者掌握飲食、運(yùn)動(dòng)、用藥、體征監(jiān)測(cè)等多方面的知識(shí)和技能。因此,本論文引入語義技術(shù),借助語義技術(shù)強(qiáng)大的知識(shí)表達(dá)能力和推理能力,通過對(duì)慢性病自我管理的內(nèi)容和過程建模,實(shí)現(xiàn)根據(jù)患者特性為慢性病患者提供個(gè)性化的治療方案和服務(wù),從而提高慢性病自我管理實(shí)施的可操作性和有效性。本論文根據(jù)慢性病自我管理的實(shí)際需求,設(shè)計(jì)和構(gòu)建了慢性病自我管理知識(shí)庫(kù),并在該知識(shí)庫(kù)的基礎(chǔ)上實(shí)現(xiàn)了移動(dòng)端的慢性病自我管理系統(tǒng)。該系統(tǒng)以慢性病自我管理過程中所需的各領(lǐng)域知識(shí)為理論依據(jù),結(jié)合語義化的患者臨床數(shù)據(jù),為患者制定個(gè)性化的治療目標(biāo)和計(jì)劃,并推薦個(gè)性化的執(zhí)行方案。系統(tǒng)通過語義推理分析患者治療中產(chǎn)生的語義化過程數(shù)據(jù),查找患者不依從或依從性差的原因,并提出相應(yīng)的應(yīng)對(duì)措施,從而提高患者的治療依從率。該系統(tǒng)通過指導(dǎo)慢性病患者進(jìn)行自我管理,幫助患者克服自我管理過程中遇到的困難,以達(dá)到促使患者長(zhǎng)期堅(jiān)持自我管理的目的。
[Abstract]:With the development of industrialization and urbanization, the accelerated process of population aging and the change of life style, the prevalence of chronic diseases is gradually increasing. The growing number of people with chronic diseases brings great financial burden and health service challenges. Strengthening health education for patients with chronic diseases and improving their self-management ability can effectively control the condition of chronic diseases, delay the occurrence of complications, reduce the economic burden of patients, and improve the ratio of medical input and output. Improve the status of chronic disease prevention and treatment. In order to manage chronic diseases well, it is necessary for the patients to know their own disease situation, and to master the knowledge and skills of diet, exercise, medication, physical sign monitoring and so on. Therefore, this paper introduces the semantic technology, with the help of the powerful knowledge expression and reasoning ability of semantic technology, by modeling the content and process of chronic disease self-management. In order to improve the operability and effectiveness of self-management of chronic diseases, individualized treatment schemes and services can be provided according to the characteristics of patients with chronic diseases. According to the actual demand of chronic disease self-management, this paper designs and constructs the chronic disease self-management knowledge base, and realizes the mobile side chronic disease self-management system based on the knowledge base. The system is based on the knowledge of various fields needed in the process of chronic disease self-management, combined with the semantic patient clinical data, to formulate individualized treatment goals and plans for patients, and to recommend individualized execution scheme. The system analyzes the semantic process data of patients through semantic reasoning, finds out the causes of patients' non-compliance or poor compliance, and puts forward the corresponding countermeasures to improve the rate of patients' treatment compliance. The system can help the patients with chronic diseases to overcome the difficulties in the process of self-management by guiding them to self-management, so as to promote the patients to adhere to self-management for a long time.
