文章摘要：目的探討一種科學的鎮(zhèn)痛、鎮(zhèn)靜管理方法，以確保ICU鎮(zhèn)痛、鎮(zhèn)靜治療的安全實施。方法2012年1～12月采用醫(yī)護合作策略，包括醫(yī)護共同參與培訓、共同決策實施方法、采用標準化溝通方式及共同討論差錯隱患等，對鎮(zhèn)痛、鎮(zhèn)靜治療進行安全管理，比較應用該策略前后的效果。結(jié)果應用醫(yī)護合作管理策略后．醫(yī)生、護士合作狀況平均得分及程序化鎮(zhèn)痛、鎮(zhèn)靜治療的落實率均高于實施前，差異有統(tǒng)計學意義（P〈0．01）。鎮(zhèn)痛、鎮(zhèn)靜不足導致氣管插管和氣切套管非計劃拔管發(fā)生率低于實施前，差異有統(tǒng)計學意義（P〈0．05）。鎮(zhèn)痛、鎮(zhèn)靜過度導致呼吸機相關性肺炎的感染率低于實施前，差異無統(tǒng)計學意義（P〉0．05），結(jié)論醫(yī)護合作策略在鎮(zhèn)痛、鎮(zhèn)靜安全管理中具有優(yōu)勢作用．ICU護士在實施過程中具有不可替代的地位，

Abstr：Objective To explore a scientific management method for patients treated with sedalives and analgesics in the Intensive Care Units（ICU）,in order to ensure patient safety. Methods Physician-nurse cooperative style was applied in the managenmnt of patients who received sedatives and analgesics from January to December 2012. The measures included co-training of physicians and nurses, joint decision-making on medication administration, implementation of procedural sedation and analgesia（PSA）, and diseussion on the risks of medical etTors tngether. Results After implementing physieian-nurse cooperative methods, the stoics of physician-nurse collal）oration and the rate nf PSA increased signifieantly （P〈0.01） ; the ineidenee of unplanned extuhation （UE） de-creased signifieantly（P〈0.05）. Conclusion The application of physician-nurse cooperative style can effectively impruve the quality of sedation and analgesia and ensure patient safety. ICU nurses play an important role in the implementation of sedation and analgesia.
