【摘要】：本文屬國(guó)家自然科學(xué)基金面上項(xiàng)目“防控突發(fā)性傳染病的醫(yī)療建筑網(wǎng)絡(luò)體系構(gòu)建”(51078104)的重要組成部分。所謂突發(fā)性傳染病,指的是在短期內(nèi)發(fā)生、有較大波及范圍,出現(xiàn)大量的病人或死亡病例,發(fā)病率遠(yuǎn)超過常年發(fā)病率水平的一種傳染病疫情;因其意外性、高危害性、廣泛影響力等特點(diǎn),已經(jīng)成為了目前人類所面臨的醫(yī)學(xué)與社會(huì)的雙重難題;考驗(yàn)著一個(gè)國(guó)家、地區(qū)、城市的公共衛(wèi)生服務(wù)體系,檢驗(yàn)著醫(yī)療建筑、醫(yī)療設(shè)施的發(fā)展均衡性。本研究從醫(yī)療建筑網(wǎng)絡(luò)化的角度出發(fā),對(duì)防控突發(fā)性傳染病的醫(yī)療建筑網(wǎng)絡(luò)體系下的醫(yī)療建筑規(guī)劃和建筑設(shè)計(jì)進(jìn)行研究,目的在于提出理論性的指導(dǎo)對(duì)策,并針對(duì)性的建構(gòu)出防控突發(fā)性傳染病的醫(yī)療建筑體系的具體操作模式。同時(shí)促進(jìn)國(guó)家“堅(jiān)持公益性、調(diào)動(dòng)積極性”的新醫(yī)改政策在疾病防控領(lǐng)域的具體應(yīng)用,在新醫(yī)改政策的框架下完善當(dāng)前的醫(yī)療建筑體系,完善醫(yī)療建筑設(shè)計(jì)方法。其最終的社會(huì)意義是探討“以人為本”的從建筑領(lǐng)域出發(fā)解決社會(huì)問題的新途徑。本文的具體研究指向防控突發(fā)性傳染病的醫(yī)療建筑網(wǎng)絡(luò)體系,以體系中最為關(guān)鍵的網(wǎng)絡(luò)結(jié)構(gòu)為關(guān)注點(diǎn),輔以系統(tǒng)論、協(xié)同論、區(qū)域科學(xué)理論等方法,創(chuàng)新性的提出了真實(shí)空間網(wǎng)絡(luò)、虛擬空間網(wǎng)絡(luò)、中介空間網(wǎng)絡(luò)三位一體的防控突發(fā)性傳染病的醫(yī)療建筑網(wǎng)絡(luò)架構(gòu),并用網(wǎng)絡(luò)配置計(jì)劃對(duì)組成網(wǎng)絡(luò)的醫(yī)療建筑要素進(jìn)行具體的規(guī)劃與設(shè)計(jì)指引,用網(wǎng)絡(luò)運(yùn)行機(jī)制對(duì)網(wǎng)絡(luò)架構(gòu)中錯(cuò)綜復(fù)雜的內(nèi)部關(guān)系進(jìn)行總結(jié)。研究循著從點(diǎn)到面的思路,從“中觀”層面構(gòu)成網(wǎng)絡(luò)的醫(yī)療建筑要素的研究介入,向上拓展到城市規(guī)劃層面的醫(yī)療建筑網(wǎng)絡(luò)體系整合,向下則具象到保證醫(yī)療建筑網(wǎng)絡(luò)體系順暢運(yùn)行的相關(guān)技術(shù)支撐,全方位的對(duì)醫(yī)療建筑網(wǎng)絡(luò)體系進(jìn)行了梳理。在防控突發(fā)性傳染病的醫(yī)療建筑網(wǎng)絡(luò)要素的研究中,選取了整個(gè)網(wǎng)絡(luò)體系中最為關(guān)鍵的四類醫(yī)療建筑“結(jié)點(diǎn)”:疾病預(yù)防控制中心、傳染病�？漆t(yī)院、綜合醫(yī)院傳染病防控空間以及基層傳染病防控醫(yī)療機(jī)構(gòu),通過對(duì)近年來大量相關(guān)案例的分析,對(duì)這四類醫(yī)療建筑在網(wǎng)絡(luò)中的職能、發(fā)展趨勢(shì)以及設(shè)計(jì)要點(diǎn)進(jìn)行梳理與闡述。在防控突發(fā)性傳染病的醫(yī)療建筑網(wǎng)絡(luò)整合模式的探索中,從政策及策略層面提出以防控突發(fā)性傳染病的城市醫(yī)療建筑規(guī)劃專篇為目標(biāo)的垂直整合模式,主要體現(xiàn)為規(guī)劃功能、規(guī)劃技術(shù)與規(guī)劃程序的整合。進(jìn)而在具體方法及手段層面描述了以“聯(lián)動(dòng)”為特質(zhì)的醫(yī)療建筑水平整合模式,并主要以哈爾濱市防控突發(fā)性傳染病的城市醫(yī)療建筑規(guī)劃專篇為例說明了這一特質(zhì)在真實(shí)空間網(wǎng)絡(luò)、虛擬空間網(wǎng)絡(luò)、中介空間網(wǎng)絡(luò)三個(gè)不同網(wǎng)絡(luò)架構(gòu)上的體現(xiàn)。在防控突發(fā)性傳染病的醫(yī)療建筑網(wǎng)絡(luò)技術(shù)支撐的闡釋中,以突發(fā)性傳染病疫情的全過程為線索,首先從廣泛分析、精確控制和優(yōu)化救治三個(gè)角度探索了技術(shù)支撐平臺(tái)的構(gòu)建方式;其次又從一體化預(yù)防、協(xié)同化控制和應(yīng)急化救治三方面來明晰具體的技術(shù)支撐策略;最后還從相應(yīng)的醫(yī)療技術(shù)方面,探索了其對(duì)于網(wǎng)絡(luò)體系的輔助支持。
[Abstract]:This is the National Natural Science Foundation Project "medical building network system for the prevention and control of emerging infectious diseases in the construction of" (51078104). This study from the medical building network perspective, research on medical construction planning and construction design of medical building network system for the prevention and control of emerging infectious diseases under the purpose of providing theoretical guidance Countermeasures and specific operation mode of prevention and control of emerging infectious diseases in the medical building system of construction. In the study of medical building network elements of prevention of sudden infectious diseases in the selected four kinds of medical building "key nodes" for the entire network in the system of center for Disease Control and prevention, infectious disease hospital, space disease prevention and control of infectious disease prevention and control and grassroots medical institutions of infectious hospital, through the Analysis in recent years a large number of cases, for the four types of medical building functions in the network, combing with the described development trend and design points.
