【摘要】：改革開放以來，隨著經(jīng)濟(jì)的不斷發(fā)展，人民生活水平得到了很大提高，與此同時(shí)中國農(nóng)村經(jīng)濟(jì)社會(huì)快速發(fā)展起來，農(nóng)民收入水平不斷提高，生活水平顯著提高，對(duì)醫(yī)療衛(wèi)生水平的需求不斷提高。為了滿足廣大農(nóng)民的衛(wèi)生需求，，我國自2002年宣布實(shí)行新型農(nóng)村合作醫(yī)療，2004年起新型農(nóng)村合作醫(yī)療在全國開始進(jìn)行試點(diǎn)工作。被調(diào)查地區(qū)沂水縣自2004年開始實(shí)行新型農(nóng)村合作醫(yī)療，經(jīng)過十年的發(fā)展，沂水縣基本建立起比較完善的新型農(nóng)村合作醫(yī)療體系，參合人數(shù)與參合比例不斷提高，醫(yī)療衛(wèi)生水平不斷提高，但仍有一部分農(nóng)民由于種種原因沒有參與或不愿參與新型農(nóng)村合作醫(yī)療。研究沂水縣新型農(nóng)村合作醫(yī)療存在的問題與農(nóng)民的參與意愿對(duì)于本縣新型農(nóng)村合作醫(yī)療的健康發(fā)展具有重大意義。 本文首先對(duì)有關(guān)合作醫(yī)療發(fā)展歷史、農(nóng)村合作醫(yī)療發(fā)展模式以及農(nóng)民參與意愿等方面的文獻(xiàn)進(jìn)行了回顧，界定了合作醫(yī)療以及新型農(nóng)村合作醫(yī)療等相關(guān)概念。從公共產(chǎn)品理論、社會(huì)保險(xiǎn)理論等幾個(gè)方面闡述了本文研究的理論基礎(chǔ)，同時(shí)建立了農(nóng)民對(duì)新型農(nóng)村合作醫(yī)療參與意愿影響因素的實(shí)證分析框架。其次通過問卷調(diào)查的方式收集了沂水縣部分農(nóng)民的數(shù)據(jù)，運(yùn)用SPSS軟件的描述性統(tǒng)計(jì)分析、因子分析方法對(duì)影響農(nóng)民參與新型農(nóng)村合作醫(yī)療的因素進(jìn)行研究。 研究結(jié)果表明：新型農(nóng)村合作醫(yī)療的管理機(jī)構(gòu)存在籌資成本過高和管理成本過高、管理人員素質(zhì)偏低等問題，新型農(nóng)村合作醫(yī)療的定點(diǎn)醫(yī)療機(jī)構(gòu)存在資金缺乏、人才缺乏和人員行為不規(guī)范等問題，而新型農(nóng)村合作醫(yī)療制度的報(bào)銷和補(bǔ)償制度也存在一些問題，農(nóng)民自身也存在健康意識(shí)淡薄、法律意識(shí)淡薄等問題，正是由于這些問題綜合起來對(duì)新型農(nóng)村合作醫(yī)療的發(fā)展產(chǎn)生不利影響。最后，本文還對(duì)沂水縣新型農(nóng)村合作醫(yī)療發(fā)展提出了一些合理建議：首先，不斷完善新型農(nóng)村合作醫(yī)療的制度建設(shè)，完善相關(guān)法律法規(guī)、完善籌資機(jī)制、完善管理體制和新農(nóng)合自身建設(shè)；其次不斷提高新農(nóng)合的定點(diǎn)醫(yī)療機(jī)構(gòu)服務(wù)水平，加強(qiáng)醫(yī)療隊(duì)伍建設(shè)和硬件設(shè)施建設(shè)；最后，不斷提高農(nóng)民的科學(xué)文化素質(zhì)和身體健康素質(zhì)
[Abstract]:Since the reform and opening up, with the continuous development of the economy, the living standards of the people have been greatly improved. At the same time, China's rural economy and society have developed rapidly, the level of peasants' income has been constantly rising, and the standard of living has improved significantly. The demand for medical and health care is constantly rising. In order to meet the health needs of farmers, China announced the implementation of the new rural cooperative medical system in 2002. In 2004, the new rural cooperative medical system began to carry out pilot work in the whole country. The new rural cooperative medical system has been implemented in Yishui County since 2004. After ten years of development, Yishui County has basically established a relatively perfect new rural cooperative medical system, and the number of participants and the proportion of participation in the cooperative medical system have been increasing. The level of medical and health care is improving, but there are still some farmers who do not participate or do not want to participate in the new rural cooperative medical system for various reasons. It is of great significance for the healthy development of the new rural cooperative medical system to study the problems and farmers' willingness to participate in the new rural cooperative medical system in Yishui county. In this paper, the development history of cooperative medical system, the development model of rural cooperative medical system and farmers' willingness to participate are reviewed, and the concepts of cooperative medical system and new rural cooperative medical system are defined. This paper expounds the theoretical basis of this study from the aspects of public product theory and social insurance theory, and establishes an empirical analysis framework of the influencing factors of farmers' willingness to participate in the new rural cooperative medical system. Secondly, the data of some farmers in Yishui County are collected by questionnaire, and the factors influencing farmers' participation in the new rural cooperative medical system are studied by using the descriptive statistical analysis of SPSS software and the method of factor analysis. The research results show that the new rural cooperative medical system has some problems, such as too high financing cost, too high management cost, low quality of management personnel, and lack of funds in designated medical institutions of the new rural cooperative medical system. There are also some problems in the reimbursement and compensation system of the new rural cooperative medical system, and there are also some problems in the farmers' own health awareness and legal awareness. It is because of these problems that the development of the new rural cooperative medical system has a negative impact. Finally, this paper also put forward some reasonable suggestions to the development of new rural cooperative medical system in Yishui county: first, to improve the system construction of the new rural cooperative medical system, to perfect the relevant laws and regulations, and to perfect the financing mechanism. Perfecting the management system and the construction of NCMS itself; Secondly, to improve the service level of the designated medical institutions of NCMS, to strengthen the construction of medical team and hardware facilities; finally, to continuously improve the scientific and cultural quality and physical health quality of farmers
