【摘要】：新型農(nóng)村合作醫(yī)療制度(以下簡稱“新農(nóng)合制度”)是一項由政府組織、引導(dǎo)、支持,農(nóng)民自愿參加,個人、集體和政府多方籌資,以大病統(tǒng)籌為主的農(nóng)民醫(yī)療互助共濟制度,是目前我國保障農(nóng)村居民醫(yī)療衛(wèi)生水平的重要制度。它有利于農(nóng)民利益得到實現(xiàn),防止農(nóng)民因病致貧或因病返貧,對于我國農(nóng)村醫(yī)療衛(wèi)生事業(yè)的發(fā)展具有重要的意義。我國的農(nóng)村合作醫(yī)療制度由來已久,經(jīng)歷了從20世紀(jì)50年代以來的風(fēng)風(fēng)雨雨,經(jīng)歷過興盛,也經(jīng)歷過衰退,但都沒有阻止其前進的步伐。新農(nóng)合制度的建立是我國各級政府對我國農(nóng)村衛(wèi)生事業(yè)不斷探索與嘗試的結(jié)果,保證新農(nóng)合制度的有效實施與可持續(xù)發(fā)展至關(guān)重要,政府作為新農(nóng)合制度的實施主體責(zé)無旁貸。 鄉(xiāng)鎮(zhèn)政府作為與農(nóng)民最接近的一級政府,對農(nóng)村與農(nóng)民的各項情況都相對的熟悉,在新農(nóng)合制度的實施中發(fā)揮著關(guān)鍵作用。鄉(xiāng)鎮(zhèn)政府在新農(nóng)合制度的實施中主要承擔(dān)著新農(nóng)合政策宣傳、組織實施的責(zé)任、籌集新農(nóng)合資金中農(nóng)民應(yīng)繳款的責(zé)任、協(xié)調(diào)各方關(guān)系、監(jiān)督管理、保障新農(nóng)合制度有效實施的責(zé)任,鄉(xiāng)鎮(zhèn)政府應(yīng)該明確自身在新農(nóng)合制度實施中的責(zé)任。但是,目前鄉(xiāng)鎮(zhèn)政府在新農(nóng)合制度的實施中仍然存在責(zé)任缺失的問題,制約了新農(nóng)合制度的發(fā)展進程,不利于新農(nóng)合制度的有效、可持續(xù)發(fā)展。 本文針對新農(nóng)合制度以及新農(nóng)合制度實施中鄉(xiāng)鎮(zhèn)政府的責(zé)任缺失問題展開研究,全文主要分為六個章節(jié)：第一章緒論,在緒論中闡述了選題的背景及意義,列舉了國內(nèi)外的研究現(xiàn)狀,并進一步引出本文所要研究的重點問題,介紹了研究本文的理論依據(jù),從公共服務(wù)理論、政府職能理論、公共產(chǎn)品供給理論以及責(zé)任政府理論提出了研究本文的依據(jù)；第二章主要關(guān)于我國農(nóng)村合作醫(yī)療的發(fā)展歷程展開線索式的介紹,并對新農(nóng)合制度的建立、內(nèi)涵、原則、主要特征等進行了分析,為下文的研究提供支持；第三章與第四章,主要從新農(nóng)合制度實施中鄉(xiāng)鎮(zhèn)政府承擔(dān)的具體責(zé)任出發(fā),先是鄉(xiāng)鎮(zhèn)政府應(yīng)該承擔(dān)的責(zé)任列舉,然后從實際情況出發(fā),闡述了新農(nóng)合制度實施中鄉(xiāng)鎮(zhèn)政府存在責(zé)任缺失的問題,如新農(nóng)合制度的宣傳不到位、資金籌集過程中存在道德風(fēng)險、新農(nóng)合制度監(jiān)管不到位、新農(nóng)合制度實施中協(xié)調(diào)溝通不足,以及鄉(xiāng)鎮(zhèn)政府工作人員的思想意識、管理理念存在問題等,并且進一步分析了鄉(xiāng)鎮(zhèn)政府責(zé)任缺失的原因,如鄉(xiāng)鎮(zhèn)政府的權(quán)責(zé)不對稱、績效考核機制不健全、監(jiān)管機制并不健全等；第五章,主要針對以上提出的鄉(xiāng)鎮(zhèn)政府在新農(nóng)合制度實施中的責(zé)任缺失以及責(zé)任缺失的原因,提出相應(yīng)的對策,來優(yōu)化鄉(xiāng)鎮(zhèn)政府在新農(nóng)合質(zhì)實施中的責(zé)任,如加大新農(nóng)合的宣傳力度、完善鄉(xiāng)鎮(zhèn)政府的績效考核標(biāo)準(zhǔn)、合理劃分鄉(xiāng)鎮(zhèn)政府的事權(quán)與財權(quán)、提升鄉(xiāng)鎮(zhèn)工作人員的素質(zhì)與能力等方面展開闡述。 總之,要完善鄉(xiāng)鎮(zhèn)政府在新農(nóng)合制度的實施中責(zé)任,這需要長時間的不斷探索與研究,也需要鄉(xiāng)鎮(zhèn)政府自身在實際工作中不斷的總結(jié)經(jīng)驗、反省思考,注重與實施對象的溝通與交流,努力向著更好的方向發(fā)展。本文的研究只是關(guān)于鄉(xiāng)鎮(zhèn)政府在新農(nóng)合制度中責(zé)任問題研究的冰山一角,具有很大的局限性,但還是希望能為鄉(xiāng)鎮(zhèn)政府在新農(nóng)合制度的實施中更好地履行其責(zé)任提供一點參考。
[Abstract]:The new rural cooperative medical system (hereinafter referred to as "the new rural cooperative medical system") is a government organization, guidance, support, the farmers' voluntary participation, individual, collective and government fund-raising, and the farmer's medical mutual aid system based on a large disease overall, is an important system to safeguard the medical and health level of rural residents in China. It is beneficial to the farmers. It is of great significance for the rural medical and health development to realize the realization of the benefits, prevent the farmers from getting poor or to return to poverty in the light of disease. The rural cooperative medical system has a long history, experienced the wind and rain since 1950s, experienced prosperity, and has experienced recession, but it has not stopped the pace of its progress. The establishment of the system of agricultural cooperation is the result of the continuous exploration and trial of China's rural health services at all levels of government. It is essential to ensure the effective implementation and sustainable development of the new rural cooperative system, and the government as the main body of the implementation of the new rural cooperative system is duty bound.
The township government, as the closest government with the peasants, is relatively familiar with the situation of rural and farmers, and plays a key role in the implementation of the new rural cooperative system. In the implementation of the new rural cooperative system, the township government is mainly responsible for the promotion of the new rural cooperative policy, the responsibility of organizing the implementation of the new rural cooperation, and raising the contribution of the farmers to the new rural cooperative funds. Responsibility, coordination of all parties, supervision and management, and the responsibility of ensuring the effective implementation of the new rural cooperative system, the township government should clarify its responsibilities in the implementation of the new rural cooperative system. However, at present, the township government still has the problem of lack of responsibility in the implementation of the new rural cooperative system, which has made the development process of the new rural cooperative system and is not conducive to the new rural cooperative system. The efficiency of the degree and the sustainable development.
This paper is aimed at the study of the lack of responsibility of the township government in the new rural cooperative system and the implementation of the new rural cooperative system. The full text is divided into six chapters: the first chapter is the introduction. In the introduction, the background and significance of the topic are expounded, the status of the research at home and abroad is enumerated, and the key problems to be studied in this paper are further introduced and the research is introduced. The theoretical basis of this article is based on the theory of public service, the theory of government function, the theory of public product supply and the theory of responsible government. The second chapter mainly introduces the clue of the development course of rural cooperative medical care in China, and has carried out the establishment, connotation, principle and main characteristics of the new rural cooperative medical system. Analysis, to provide support for the following research; the third chapter and the fourth chapter, mainly from the implementation of the specific responsibilities of the township government in the implementation of the NCMS system, first of all the responsibilities of the township government should be listed, and then from the actual situation, the problem of the lack of responsibility for the existence of the township government in the implementation of the new rural cooperative system, such as the new agricultural cooperation system, is expounded. The publicity is not in place, there is moral hazard in the process of fund raising, the supervision of the new rural cooperative system is not in place, the coordination and communication of the new rural cooperation system are not enough, the ideological consciousness of the staff of the township government and the problems of the management idea exist, and the reasons for the lack of responsibility for the township government are further analyzed, such as the right and responsibility of the township government is not right. It is said that the mechanism of performance assessment is not sound and the regulatory mechanism is not sound. The fifth chapter mainly aims at the lack of responsibility and the reasons for the lack of responsibility in the implementation of the new rural cooperative system by the township government above, to optimize the responsibility of the township government in the implementation of the new farmers' quality, such as increasing the publicity and improvement of the new rural cooperative system. The performance appraisal standards of township governments, the division of power and financial rights of township governments, and the improvement of the quality and ability of township staff are expounded.
In a word, it is necessary to improve the responsibility of the township government in the implementation of the new rural cooperative system. It needs a long time to explore and study, and it also needs the township government to constantly summarize the experience in the actual work, reflect on it, pay attention to the communication and communication with the implementation object, and try to develop in a better direction. This study is only about the township government. The iceberg in the study of the responsibility of the government in the new rural cooperative system has great limitations, but it still hopes to provide a reference for the township government to better perform its responsibility in the implementation of the new rural cooperative system.
