腫瘤方向護(hù)理碩士專業(yè)學(xué)位研究生臨床實(shí)踐培養(yǎng)模式的研究

【摘要】:目的:以崗位勝任力為理論依據(jù),借鑒國外護(hù)理研究生的培養(yǎng)經(jīng)驗(yàn),參考住院醫(yī)師、新入職護(hù)士的規(guī)范化培養(yǎng),構(gòu)建具有腫瘤護(hù)理特色的護(hù)理碩士專業(yè)學(xué)位研究生臨床實(shí)踐培養(yǎng)模式,明確腫瘤方向護(hù)理碩士專業(yè)學(xué)位研究生的培養(yǎng)目標(biāo),細(xì)化臨床實(shí)踐的內(nèi)容與要求,培養(yǎng)與臨床接軌的應(yīng)用型護(hù)理碩士專業(yè)學(xué)位研究生,促進(jìn)我國高層次腫瘤?谱o(hù)士的培養(yǎng)和腫瘤專科的發(fā)展。方法:運(yùn)用文獻(xiàn)分析法并參考《實(shí)用腫瘤護(hù)理學(xué)》第二版(人民衛(wèi)生出版社出版,主編聞曲),初步擬定腫瘤方向護(hù)理碩士專業(yè)學(xué)位研究生臨床實(shí)踐培養(yǎng)模式問卷,經(jīng)過半結(jié)構(gòu)訪談法,預(yù)調(diào)查對問卷的內(nèi)容進(jìn)行增減,修改措辭,形成第一輪專家函詢問卷。選取在腫瘤醫(yī)療、護(hù)理教育、護(hù)理管理、臨床護(hù)理領(lǐng)域有經(jīng)驗(yàn)的25名專家進(jìn)行兩輪德爾菲(Delphi)函詢,采用Excel 2016和SPSS 17.0軟件進(jìn)行統(tǒng)計(jì)分析,將兩輪函詢結(jié)果整合修改,最終形成腫瘤方向護(hù)理碩士專業(yè)學(xué)位研究生臨床實(shí)踐培養(yǎng)模式,并對培養(yǎng)模式的一、二級條目采用百分權(quán)重法和乘積法進(jìn)行權(quán)重計(jì)算并分析。結(jié)果:1.本研究德爾菲法(Delphi法)共進(jìn)行兩輪,兩輪發(fā)放問卷數(shù)分別為30份、25份,有效回收問卷數(shù)分別為25份、23份,專家積極系數(shù)分別為83.33%、92.00%,兩輪咨詢中共有27名專家給予建議及意見,專家積極程度較高。25名專家的學(xué)術(shù)水平系數(shù)為0.8880、判斷依據(jù)系數(shù)為0.9160、熟悉程度系數(shù)為0.8440以及權(quán)威系數(shù)為0.8827,專家的權(quán)威程度較高。兩輪函詢總體Kendall'sω分別為0.215、0.247(P0.01),專家對各條目的認(rèn)可程度較高;2.四個(gè)一級條目權(quán)重大小順位排序分別為:實(shí)踐方法(0.2532)、實(shí)踐內(nèi)容與要求(0.2532)、實(shí)踐目標(biāo)(0.2506)、考核評價(jià)(0.2429);15個(gè)2級條目的權(quán)重大小排在前5位的分別是:知識(shí)(0.0181)、技能(0.0177)、?普n程(0.0175)、病種護(hù)理及例數(shù)(0.0175)、操作技術(shù)及例數(shù)(0.0175);3.兩輪問卷函詢結(jié)束后,形成腫瘤方向護(hù)理碩士專業(yè)學(xué)位研究生臨床實(shí)踐培養(yǎng)模式,包含4個(gè)一級條目,15個(gè)二級條目,153個(gè)三級條目。結(jié)論:經(jīng)過兩輪專家函詢,將專家建議與意見整理修改后,形成腫瘤方向護(hù)理碩士專業(yè)學(xué)位研究生臨床實(shí)踐培養(yǎng)模式,包含實(shí)踐目標(biāo)、實(shí)踐方法、實(shí)踐內(nèi)容與要求和考核評價(jià)在內(nèi)的4個(gè)一級條目,15個(gè)二級條目,153個(gè)三級條目,專家積極性較高,權(quán)威程度好,研究結(jié)果可靠。本研究成果已在我校護(hù)理碩士專業(yè)學(xué)位研究生的培養(yǎng)中實(shí)施,但由于初次實(shí)踐,還需在后期實(shí)施中進(jìn)行動(dòng)態(tài)監(jiān)測和調(diào)節(jié)。
[Abstract]:Objective: based on the theory of post competence, to learn from the training experience of foreign nursing graduate students, to refer to the standardized training of resident doctors and new nurses. The cultivation model of clinical practice of nursing master degree graduate students with characteristics of tumor nursing should be established, and the cultivation goal of nursing master degree graduate students in cancer direction should be clarified, and the contents and requirements of clinical practice should be refined. The cultivation of applied nursing master's degree, which is in line with clinical practice, promotes the cultivation of high-level oncology nurses and the development of oncology specialty in China. Methods: using the method of literature analysis and referring to the second edition of practical Cancer Nursing (published by people's Health Publishing House and editor-in-chief Wen qu), the questionnaire of clinical practice cultivation model for graduate students majoring in cancer direction nursing was preliminarily drawn up. After the semi-structured interview, the content of the questionnaire was added or decreased, the wording was revised, and the first round of questionnaire was formed. A total of 25 experts with experience in the field of cancer medicine, nursing education, nursing management and clinical nursing were selected to conduct two rounds of Delphi mail inquiry. The results of the two rounds of inquiry were analyzed by Excel 2016 and SPSS 17.0 software. Finally, the cultivation model of clinical practice was formed for the graduate students majoring in tumor direction nursing, and the weights of the first and second grade items were calculated and analyzed by the percentage weight method and the product method. The result is 1: 1. Two rounds of Delphi method were carried out in this study. The number of questionnaires sent out in the two rounds was 30 or 25, the number of valid questionnaires was 25 or 23, the positive coefficient of experts was 83.33 and 92.000.27 experts were consulted in the two rounds to give suggestions and opinions. The academic level coefficient of 25 experts is 0.8880, the judging basis coefficient is 0.9160, the familiarity coefficient is 0.8440 and the authority coefficient is 0.8827. The total Kendall's 蠅 of the two rounds of letters is 0. 215 / 0.247 / P0. 01 respectively, and the experts have a higher degree of approval of each item. The order of weight of the four first-level items is: practice method 0.2532, practice content and requirement 0.2532, practice goal 0.2506, assessment 0.2429; the weight of 15 second-level items in the top 5 are: knowledge 0.0181, skill 0.0177and specialized course. The nursing of the disease and the number of cases were 0.0175%, and the operation technique and the number of cases were 0.0175%. After two rounds of questionnaire, the clinical practice cultivation model of tumor direction nursing master degree graduate students was formed, which included 4 first grade items, 15 second grade items and 153 third grade items. Conclusion: after two rounds of experts' inquiry, the experts' suggestions and opinions were arranged and revised to form the clinical practice training model for the graduate students majoring in tumor direction nursing, including the practical objectives and methods. There are 4 first-level items, 15 second-level items and 153 third-level items, including practical contents, requirements and evaluation. The experts' enthusiasm is high, the degree of authority is good, and the research results are reliable. The results of this study have been carried out in the cultivation of nursing master's degree graduate students in our university, but because of the initial practice, it is necessary to carry out dynamic monitoring and adjustment in the later stage of implementation.
