【摘要】：目的探討不同濃度氯己定口腔護(hù)理液在經(jīng)口氣管插管患者中的應(yīng)用效果。方法2012年1月—2013年12月某院綜合ICU收治的經(jīng)口氣管插管行機(jī)械通氣時(shí)間48 h以上患者,共納入120例符合條件患者,隨機(jī)分為3組,每組各40例。使用不同濃度氯己定口腔護(hù)理液應(yīng)用于各組患者:試驗(yàn)組(2%)、對(duì)照組Ⅰ(0.2%)、對(duì)照組Ⅱ(0.12%)。觀察并比較3組患者在口臭、口腔黏膜感染、呼吸機(jī)相關(guān)肺炎(VAP)發(fā)生時(shí)間及VAP發(fā)病率的差異。結(jié)果試驗(yàn)組與對(duì)照組Ⅰ、對(duì)照組Ⅱ患者的VAP、早發(fā)VAP的發(fā)病率比較差異有統(tǒng)計(jì)學(xué)意義(均P0.05);進(jìn)一步兩兩比較,對(duì)照組Ⅱ患者VAP發(fā)病率高于試驗(yàn)組(47.50%VS 20.00%,P=0.009)。結(jié)論應(yīng)用2%氯己定與有效的沖洗加擦拭口腔護(hù)理方法可有效降低經(jīng)口氣管插管患者的VAP發(fā)病率。
