【摘要】：目的調(diào)查一組耐藥陰溝腸桿菌中可移動(dòng)耐藥元件(可移動(dòng)遺傳元件遺傳標(biāo)記與耐藥基因)的攜帶情況,以及本組菌株的親緣關(guān)系。方法收集醫(yī)院2015年1至12月住院病人痰液標(biāo)本中分離的耐藥陰溝腸桿菌共20株,采用聚合酶鏈反應(yīng)(PCR)及序列分析的方法分析11種可移動(dòng)遺傳元件與20種β-內(nèi)酰胺類(lèi)耐藥基因、16種氨基糖苷類(lèi)耐藥基因。并對(duì)結(jié)果進(jìn)行了樣本聚類(lèi)分析。結(jié)果有19株陰溝腸桿菌檢出可移動(dòng)遺傳元件遺傳標(biāo)記、β-內(nèi)酰胺類(lèi)和氨基糖苷類(lèi)耐藥基因。20株耐藥陰溝腸桿菌共檢出5種可移動(dòng)遺傳元件遺傳標(biāo)記:tnp513、tnpU、IS26、IS903、intⅠ1,4種β-內(nèi)酰胺類(lèi)耐藥基因:blaTEM、blaOXA-1群、blaDHA、blaACT/MIR群,6種氨基糖苷類(lèi)耐藥基因:aac(3)-Ⅱ、aac(6′)-Ⅰb、ant(3″)-Ⅰ、ant(2″)-Ⅰ、aph3′-Ⅵ、rmtB。樣本聚類(lèi)分析提示,除2號(hào)菌與18號(hào)菌外,其余18株顯聚集性。3-9-10-16號(hào)菌株,1-17號(hào)菌株,5-14號(hào)菌株,15-19號(hào)菌株均為克隆傳播。結(jié)論本組耐藥陰溝腸桿菌檢出的可移動(dòng)耐藥元件是導(dǎo)致對(duì)β-內(nèi)酰胺類(lèi)、氨基糖苷類(lèi)耐藥的重要原因,并且耐藥表型和基因型一一對(duì)應(yīng)。本組菌中4個(gè)不同的克隆傳播,有醫(yī)院感染的可能。
[Abstract]:Objective to investigate the carrying status of a group of drug-resistant Enterobacter cloacae (Drug-resistant strains) and the relationship between the transportable elements (transportable genetic elements genetic markers and drug-resistant genes). Methods A total of 20 strains of drug-resistant Enterobacter cloacae isolated from sputum samples of hospitalized patients from January to December 2015 were collected. Polymerase chain reaction (PCR) and sequence analysis were used to analyze 11 transportable genetic elements and 20 尾 -lactam resistant genes and 16 aminoglycoside resistant genes. The results are analyzed by sample clustering. Results A total of 19 strains of Enterobacter cloacae were found to have transportable genetic element genetic markers, and 尾 -lactam and aminoglycoside resistance genes. 20 strains of drug-resistant Enterobacter cloacae were found to have 5 transportable genetic element genetic markers: tnp513TnpUIS26TnpUIS26S903int 鈪，
