【摘要】：目的探討亞低溫療法對(duì)心肺復(fù)蘇后患者神經(jīng)功能的影響。方法采用單中心前瞻性隨機(jī)對(duì)照的臨床研究方法,選取2014年9月—2015年12月濰坊醫(yī)學(xué)院附屬醫(yī)院搶救的心搏驟停進(jìn)行心肺復(fù)蘇后自主循環(huán)恢復(fù)的患者為研究對(duì)象,按隨機(jī)數(shù)字表法將患者分為對(duì)照組和試驗(yàn)組。自主循環(huán)恢復(fù)后,對(duì)照組給予常規(guī)頭部冰袋降溫治療,試驗(yàn)組患者快速2 h體溫降至33~35℃后,應(yīng)用亞低溫治療儀維持亞低溫(33~35 V)治療。記錄復(fù)蘇后0、2、24、48、72 h患者直腸溫度、血清干擾素α(TNF-α)和白介素6(IL-6)表達(dá)水平及格拉斯哥昏迷量表(GCS)評(píng)分。結(jié)果共45例患者納入本研究,其中試驗(yàn)組23例,對(duì)照組22例。復(fù)蘇后0 h,兩組直腸溫度、TNF-α和IL-6表達(dá)水平比較,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);復(fù)蘇后2、24、48、72 h,試驗(yàn)組直腸溫度、TNF-α和IL-6表達(dá)水平均低于對(duì)照組,GCS評(píng)分均高于對(duì)照組(P0.05)。對(duì)照組和試驗(yàn)組TNF-α、IL-6表達(dá)水平與GCS評(píng)分均呈正相關(guān)(=0.683、0.969,P0.05;=0.815、0.985,P0.05)。結(jié)論心肺復(fù)蘇后患者應(yīng)用亞低溫療法可以降低血清TNF-α及IL-6表達(dá)水平,減輕組織損傷,增高GCS評(píng)分,改善患者神經(jīng)功能預(yù)后。
[Abstract]:Objective to investigate the effect of mild hypothermia therapy on neurological function after cardiopulmonary resuscitation (CPR). Methods A single center prospective randomized controlled clinical study was conducted to study the patients with cardiac arrest after cardiopulmonary resuscitation (CPR) from September 2014 to December 2015 in the affiliated Hospital of Weifang Medical College. Patients were divided into control group and trial group according to random digital table. After the recovery of spontaneous circulation, the control group was treated with conventional head ice bag cooling.
