【摘要】：目的:探討島蓋綜合征的主要臨床特征和恢復規(guī)律,優(yōu)化島蓋綜合征的臨床診斷和康復干預方法。方法:收集13例門診或住院的島蓋綜合征患者,用口面失用量表、Frenchay構(gòu)音障礙評定法等量表,在首次門診或入院后第1天、出院前1天評定患者,根據(jù)入院后第1天康復評定的結(jié)果予以住院患者個體化的康復干預。結(jié)果:8例皮質(zhì)型患者的皮質(zhì)病損部位累及單側(cè)或雙側(cè)島蓋;所有患者首診或入院時口面失用量表評定均提示有顯著的口、面、舌肌的自主運動障礙,住院患者經(jīng)康復干預后口面失用量表評分顯著性增高,9例入院時為鼻飼營養(yǎng)的住院患者經(jīng)康復治療后有6例患者改為經(jīng)口進食,約占66.7%;所有患者首診或入院時均表現(xiàn)為重度構(gòu)音障礙,10例住院患者經(jīng)20天以上的康復訓練,仍為重度構(gòu)音障礙。結(jié)論:口腔期吞咽障礙和重度構(gòu)音障礙是島蓋綜合征患者重要臨床特征,大多數(shù)島蓋綜合征患者經(jīng)康復治療能恢復經(jīng)口進食,而構(gòu)音障礙卻很難恢復。
[Abstract]:Objective: To explore the main clinical features and recovery rules of island lid syndrome and to optimize the clinical diagnosis and rehabilitation intervention methods of island cover syndrome. Methods: 13 cases of island gait syndrome were collected in outpatient or hospitalized patients, using the mouth surface loss meter, Frenchay dysarthria assessment method, and first days after the first outpatient or hospital admission, and 1 days before discharge. According to the results of the rehabilitation assessment for first days after admission, the individual rehabilitation intervention was given. Results: 8 cases of cortical lesions were involved in unilateral or bilateral Island cover, and all patients' first visit or admission to the hospital showed significant oral, facial, autonomic dyskinesia and rehabilitation of patients in the hospital. The score of the oral surface loss meter was significantly higher after the intervention, and in 9 hospitalized patients with nasogastric nutrition at admission, 6 patients were changed into oral feeding, accounting for about 66.7%. All the patients showed severe dysarthria during the first visit or admission, and 10 hospitalized patients were trained for more than 20 days and still had severe dysarthria. Conclusion: mouth Dysphagia and severe dysarthria are important clinical features of the patients with island gait syndrome. Most of the island's syndrome patients recover through rehabilitation, while dysarthria is difficult to recover.
