【摘要】：目的探討超聲心動圖診斷胎兒右位主動脈弓(RAA)及其在預(yù)后評估中的應(yīng)用價值。方法回顧性分析經(jīng)超聲心動圖診斷為胎兒RAA的25名單胎孕婦資料。7胎引產(chǎn)終止妊娠,對其中4胎進(jìn)行胎兒尸體檢查。對18胎繼續(xù)妊娠者,于產(chǎn)后1個月內(nèi)對患兒行超聲心動圖、MR或CT檢查,并臨床隨訪半年,觀察患兒有無反復(fù)發(fā)作的氣管或食管受壓癥狀。結(jié)果 19胎為RAA伴左位動脈導(dǎo)管、迷走左鎖骨下動脈,4胎為RAA伴頭臂動脈鏡像分支(3胎伴右位動脈導(dǎo)管、1胎伴左位動脈導(dǎo)管),2胎為雙主動脈弓。18胎為單純性RAA,余7胎合并其他心內(nèi)外畸形,且包括2胎染色體異常。4胎經(jīng)尸體檢查證實(shí)為RAA。18胎出生后經(jīng)超聲心動圖、MR或CT檢查證實(shí)為RAA患兒,且出生后半年內(nèi)15例患兒無明顯氣管或食管受壓癥狀,3例出現(xiàn)反復(fù)發(fā)作的慢性咳嗽、氣喘,吞咽困難,經(jīng)抗感染治療后癥狀緩解。結(jié)論超聲心動圖是診斷胎兒RAA的可靠方法;胎兒RAA的預(yù)后主要與是否并發(fā)其他心內(nèi)外畸形或染色體異常有關(guān)。
[Abstract]:Objective to evaluate the value of echocardiography in the diagnosis of fetal right aortic arch (RAA) and its prognostic evaluation.Methods the data of 25 pregnant women with fetal RAA diagnosed by echocardiography were analyzed retrospectively. Pregnancy induced pregnancy was terminated. Fetal cadavers were examined in 4 of 25 fetuses.Echocardiographic Mr or CT examination was performed on 18 children who continued pregnancy within 1 month after delivery. The children were followed up for half a year to see if they had recurrent symptoms of trachea or esophageal compression.Results 19 fetuses were RAA with left artery catheter.4 cases of left subclavian artery with RAA, 3 cases with right artery ductus arteriosus, 1 fetus with left artery ductus arteriosus, 2 cases with double aortic arch. 18 fetus are simple RAA, the remaining 7 cases are complicated with other internal and external cardiac malformations.In addition, 2 cases with chromosomal abnormalities were confirmed by cadaveric examination to be children with RAA after birth by echocardiography, Mr or CT, and were confirmed by cadaveric examination.There were no obvious symptoms of trachea or esophagus compression in 15 children within six months after birth. 3 cases had recurrent chronic cough, asthma and dysphagia, and the symptoms were relieved after anti-infection treatment.Conclusion Echocardiography is a reliable method for the diagnosis of fetal RAA, and the prognosis of fetal RAA is mainly related to other anomalies of heart and heart or chromosomal abnormalities.
