【摘要】：目的研究不同類型腦膜炎感染患兒腦脊液C-反應(yīng)蛋白(CRP)與乳酸(LA)等相關(guān)指標(biāo)變化及臨床意義,為提高確診率提供依據(jù)。方法選取2014年12月-2015年12月醫(yī)院診治的67例腦膜炎感染患兒資料進(jìn)行分析,將其設(shè)為試驗(yàn)組,選取同期入院的22例非中樞神經(jīng)系統(tǒng)疾病患兒設(shè)為對照組,采用連續(xù)監(jiān)測酶法、終點(diǎn)比色法以及免疫比濁法檢測兩組患兒腦脊液中腺苷脫氨酶(ADA)、LA、CRP、乳酸脫氫酶(LDH)的活性,并分析不同類型腦膜炎感染患兒腦脊液CRP、LA等相關(guān)指標(biāo)變化。結(jié)果試驗(yàn)組感染患兒腦脊液ADA、LA、CRP、LDH的活性指標(biāo),顯著高于對照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);不同類型腦膜炎患兒ADA、LA、CRP、LDH的活性指標(biāo)差異有統(tǒng)計(jì)學(xué)意義(P0.05);病毒性腦膜炎患兒ADA、LA、CRP、LDH的活性指標(biāo),顯著低于化膿性腦膜炎和結(jié)核性腦膜炎患兒,差異有統(tǒng)計(jì)學(xué)意義(P0.05);受試者工作特征曲線(ROC曲線)下面積大小依次為:ADA、LA、CRP、LDH;ADA指標(biāo)的敏感性、特異性,均顯著高于其他3種活性指標(biāo),差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論不同類型腦膜炎感染患兒ADA、LA、CRP、LDH的活性指標(biāo)變化不同,通過聯(lián)合ADA、LA、CRP、LDH檢測,能夠?qū)Σ煌愋偷哪X膜炎患兒進(jìn)行有效診斷,具有較高的診斷價值,值得推廣應(yīng)用。
[Abstract]:Objective to study the effect of different types of meningitis C- infection in cerebrospinal fluid of children with C-reactive protein (CRP) and lactic acid (LA) changes and other related indicators and clinical significance, and provide evidence for improving the diagnosis rate. Methods from December 2014 -2015 year in December the hospital treated 67 cases of meningitis infection were analyzed, the design for the experimental group, were selected 22 patients with non central nervous system diseases as control group, using continuous monitoring of enzymatic end point colorimetric method and immune turbidimetry detection of two groups of children with adenosine deaminase in cerebrospinal fluid (ADA), LA, CRP, lactate dehydrogenase (LDH) activity, and analysis of different types of CRP infection in cerebrospinal fluid of children with meningitis, change index of LA. The results of the experimental group of ADA infection, cerebrospinal fluid in children with LA CRP, LDH, activity index, was significantly higher than the control group, the difference was statistically significant (P0.05); different types of meningitis in children with ADA, LA, CRP, LDH activity index There are significant differences in standard (P0.05); viral meningitis in children with ADA, LA, CRP, LDH activity index, was significantly lower than that of purulent meningitis and tuberculous meningitis in children, the difference was statistically significant (P0.05); receiver operating characteristic curve (ROC curve) under large area small is as follows: ADA, LA. CRP, LDH; ADA index of sensitivity, specificity, were significantly higher than that of the other 3 active index, the difference was statistically significant (P0.05). Conclusion the different types of meningitis infection in children with ADA, LA, CRP, LDH activity index, through the combination of ADA, LA, CRP, LDH detection, can effectively diagnose the different types of meningitis, which has high diagnostic value, it is worthy of popularization and application.
