基于成熟度模型的醫(yī)療設(shè)備管理能力評(píng)估體系的構(gòu)建

【摘要】:目的:本研究借鑒JCI國(guó)際標(biāo)準(zhǔn)、ISO國(guó)際標(biāo)準(zhǔn)和等級(jí)醫(yī)院評(píng)審標(biāo)準(zhǔn)的思想,以ISO9001—2015版本作為模板構(gòu)建醫(yī)療設(shè)備管理能力評(píng)價(jià)指標(biāo)體系,并確定各指標(biāo)權(quán)重。在此基礎(chǔ)上,借鑒CMM能力成熟度的思想,運(yùn)用ISO9001質(zhì)量管理成熟度通用模型進(jìn)行醫(yī)療設(shè)備管理能力成熟度評(píng)價(jià),解決等級(jí)醫(yī)院評(píng)審和JCI評(píng)審等達(dá)標(biāo)性評(píng)審的不足和局限性,分析醫(yī)院設(shè)備管理部門設(shè)備管理水平,同時(shí)為醫(yī)院內(nèi)部評(píng)審和政府等第三方組織評(píng)價(jià)提供一定的參考依據(jù)。方法:查閱文獻(xiàn),了解醫(yī)療設(shè)備管理現(xiàn)狀及管理能力評(píng)估辦法,初步構(gòu)建醫(yī)療設(shè)備管理能力評(píng)估指標(biāo)體系;通過兩輪Delphi修改和完善醫(yī)療設(shè)備管理能力評(píng)估指標(biāo)體系;運(yùn)用AHP計(jì)算各指標(biāo)權(quán)重并賦予相應(yīng)分值;根據(jù)ISO9001質(zhì)量管理成熟度通用模型及評(píng)審指南,對(duì)每個(gè)指標(biāo)打分,并計(jì)算各指標(biāo)得分之和,即成熟度評(píng)價(jià)。利用Excel2003、SPSS l3.0軟件進(jìn)行數(shù)據(jù)處理分析。結(jié)果:1.醫(yī)療設(shè)備管理能力評(píng)估指標(biāo)體系的構(gòu)建通過兩輪專家咨詢對(duì)指標(biāo)條目進(jìn)行修改與篩選,最終構(gòu)建醫(yī)療設(shè)備管理能力評(píng)估指標(biāo)體系,包括7個(gè)一級(jí)指標(biāo):背景及環(huán)境、領(lǐng)導(dǎo)作用、核心管理流程及程序、人員能力、設(shè)備文件管理、績(jī)效評(píng)價(jià)及改進(jìn)、質(zhì)量安全文化,25個(gè)二級(jí)指標(biāo)。2.德爾菲評(píng)價(jià)指標(biāo)結(jié)果兩輪專家咨詢結(jié)果顯示,專家的積極系數(shù)為1、0.96,專家積極性較高;熟悉程度系數(shù)、判斷系數(shù)和權(quán)威系數(shù)分別為0.71、0.92、0.81,專家咨詢權(quán)威程度較高。兩輪專家咨詢的協(xié)調(diào)系數(shù)分別為0.234、0.245,專家意見協(xié)調(diào)性較好,對(duì)指標(biāo)認(rèn)可度較高。3.醫(yī)療設(shè)備管理能力評(píng)估指標(biāo)體系權(quán)重的設(shè)置通過AHP得出醫(yī)療設(shè)備管理能力評(píng)估指標(biāo)體系各條目的權(quán)重得分,其中一級(jí)指標(biāo)所的權(quán)重為背景及環(huán)境(0.0585)、領(lǐng)導(dǎo)作用(0.2285)、核心管理流程及程序(0.2786)、人員能力(0.1630)、設(shè)備文件管理(0.0455)、績(jī)效評(píng)價(jià)及改進(jìn)(0.1337)、質(zhì)量安全文化(0.0922)。4.醫(yī)療設(shè)備管理能力評(píng)估指標(biāo)體系的檢驗(yàn)指標(biāo)體系信度和效度檢驗(yàn)結(jié)果顯示,總指標(biāo)體系的分半信度為0.787,克朗巴赫系數(shù)為0.879,各一級(jí)指標(biāo)與總指標(biāo)體系的相關(guān)系數(shù)在0.627-0.819之間,指標(biāo)體系的內(nèi)容具有較好的信度和效度。5.醫(yī)療設(shè)備管理能力評(píng)估指標(biāo)體系的成熟度評(píng)價(jià)及實(shí)例分析在構(gòu)建指標(biāo)體系及設(shè)置權(quán)重的基礎(chǔ)上,提出對(duì)醫(yī)療設(shè)備管理能力進(jìn)行成熟度評(píng)價(jià),采用中國(guó)質(zhì)量協(xié)會(huì)發(fā)布的ISO9001質(zhì)量管理成熟度評(píng)價(jià)準(zhǔn)則中的成熟度評(píng)價(jià)通用模型及基本評(píng)價(jià)方法,結(jié)合具體數(shù)據(jù)進(jìn)行了實(shí)例分析。結(jié)果顯示,天津市A醫(yī)院設(shè)備管理能力成熟度總體得分為740.97,該院設(shè)備管理能力處于第四等級(jí),即管理級(jí)。結(jié)論:構(gòu)建了醫(yī)療設(shè)備管理能力評(píng)估指標(biāo)體系,并確了定指標(biāo)權(quán)重,運(yùn)用成熟度評(píng)價(jià)方法,進(jìn)行醫(yī)療設(shè)備管理能力成熟度評(píng)估的實(shí)例研究。指標(biāo)體系構(gòu)建的專家咨詢結(jié)果可靠,信度和效度較好,能夠?yàn)槲覈?guó)醫(yī)院醫(yī)療設(shè)備管理能力的評(píng)估及評(píng)價(jià)提供參考依據(jù)。
[Abstract]:Objective: based on the ideas of JCI international standard, ISO international standard and grade hospital evaluation standard, this study uses ISO9001-2015 version as template to construct the evaluation index system of medical equipment management ability, and to determine the weight of each index. On this basis, drawing lessons from the idea of CMM capability maturity, the general model of ISO9001 quality management maturity is used to evaluate the capability maturity of medical equipment management, to solve the shortcomings and limitations of standard evaluation such as grade hospital evaluation and JCI evaluation, to analyze the equipment management level of hospital equipment management department, and to provide some reference for hospital internal evaluation and government and other third-party organization evaluation. Methods: consult the literature, understand the present situation of medical equipment management and the evaluation method of management ability, initially construct the evaluation index system of medical equipment management ability, modify and perfect the evaluation index system of medical equipment management ability through two rounds of Delphi, calculate the weight of each index and give the corresponding score through two rounds of AHP. According to the general model and evaluation guide of ISO9001 quality management maturity, each index is scored and the sum of each index score is calculated, that is, maturity evaluation. The data processing and analysis are carried out by using Excel2003,SPSS l3.0 software. Result: 1. The construction of medical equipment management ability evaluation index system through two rounds of expert consultation to modify and screen the index items, and finally construct the medical equipment management ability evaluation index system, including seven first-level indicators: background and environment, leadership, core management processes and procedures, personnel capacity, equipment file management, performance evaluation and improvement, quality and safety culture, 25 secondary indicators. 2. The results of two rounds of expert consultation showed that the positive coefficient of experts was 1, 0.96, and the enthusiasm of experts was higher, and the familiarity coefficient, judgment coefficient and authority coefficient were 0.71, 0.92 and 0.81, respectively. The coordination coefficients of the two rounds of expert consultation are 0.234 and 0.245. the coordination of expert opinion is better and the degree of approval of the index is higher.
