【摘要】：全科醫(yī)療是基礎(chǔ)保健的核心,已經(jīng)成為各國(guó)高價(jià)值衛(wèi)生保健體系的基石。首診、可及性、連續(xù)性、協(xié)調(diào)性和綜合性五大特征構(gòu)成了以功能為導(dǎo)向的全科醫(yī)療定義的要素。本文從全科醫(yī)療定義出發(fā),厘清和歸納全科醫(yī)療五大特征功能的定義、內(nèi)涵及其作用,探索五大特征功能對(duì)衛(wèi)生體系高績(jī)效的作用機(jī)制。并對(duì)我國(guó)全科醫(yī)療提出建議:將基層醫(yī)療衛(wèi)生服務(wù)體系更名為基礎(chǔ)保健體系;以五大核心特征功能為重點(diǎn),發(fā)展強(qiáng)化基礎(chǔ)保健體系,促進(jìn)基層醫(yī)療衛(wèi)生服務(wù)轉(zhuǎn)型升級(jí);重點(diǎn)從全科醫(yī)生數(shù)量、薪酬制度、醫(yī)保采購(gòu)以及全科醫(yī)生教育培訓(xùn)體系等方面完善全科醫(yī)生制度;從組織層面建立有助于實(shí)施全科醫(yī)療特征功能的管理制度與服務(wù)流程。
[Abstract]:General practice is the core of basic health care and has become the cornerstone of high-value health care system. The five features of first diagnosis, accessibility, continuity, coordination and comprehensiveness constitute the essential elements of the functional-oriented definition of general practice. Starting from the definition of general practice, this paper clarifies and sums up the definition, connotation and function of the five features of general practice, and explores the mechanism of the function of the five features on the high performance of health system. Some suggestions are put forward for general medical service in China: changing the name of primary medical service system to basic health care system, developing and strengthening basic health care system and promoting the transformation and upgrading of primary medical and health service with the emphasis on five core features. It focuses on perfecting the general practitioner system from the aspects of the number of general practitioners, salary system, medical insurance procurement and general practitioner education and training system, and sets up the management system and service flow from the organizational level which is helpful to implement the general practice medical characteristic function.
